Clinical Study Of Intravenous Compound Danshen Injection Combined With Anisodamine On IgA Nephropathy Patients With Major Manifestation Being Hematuria

Objectives: To investigate the Clinical effects and the effects of hemorrheology of Compound Danshen Injection combined with Anisodamine on IgA Nephropathy (IgAN) patients with major manifestation being hematuria, and meanwhile to explore the mechanisms of combined therapy on IgAN of this type.Methods: 74 IgAN patients with major manifestation being hematuria were chosen and divided randomly into therapy group (37 cases) and control group (37 cases). Therapy group was treated by intravenous Compound Danshen Injection combined with Anisodamine. Control group was treated by intravenous Heparin Calcium and oral Polysaccharide Sodium and Persatih. Before and after therapy, urine erythrocyte count.urine protein of 24 hours, urine routine, as well as serum creatinine (Scr), plasma albumin (Alb), serum immunoglobulin (IgG, IgM, IgA) and hemorheology of all patients were tested. The effects of both groups were compared after therapy.Results: After treatment, the results of 30 patients in each group were compared. (1) Compared with hematuria and proteinuria in therapy group before and after treatment, there was significant difference (P < 0.01, P < 0.05) . Compared with those in control group, only proteinuria improved distinctly(P < 0.01, P < 0.05). Compared with those in both groups after treatment, only hematuria was remarkably different (P < 0.01). (2) After treatment, serum IgA in therapy group improved distinctly (P < 0.01) , and it was much more obvious than that in control group (P < 0.01) . (3) After treatment, all hemorheology indexes in both groups ameliorated significantly other than rigidity index of erythrocyte in contol group (P < 0.01, P < 0.05) . Compared with contol group, in therapy group, all hemorheology indexes ameliorated markedly except plasma riscosity and fibrinogen (P < 0.01, P < 0.05) . (4) TheClinical effects of therapy group was obviously superior to those of control group (P < 0.05) . Conclusion: (1) The pathological status of hypercogulabity, hemorhelogy and microcircuiation disorders occurs in IgAN patients with major manifestation being hematuria. This type IgAN is classified to Blood stasis syndrome of TCM. (2) Compound Danshen Injection combined with Anisodamine can improve microcircuiation cooperatively. Using the combined therapy to treat IgAN patients of this type can improve hypercogulabity state, hemorhelogy and microcircuiation disorders. The combined therapy can reduce the excreation of urinary erythrocyte and protein, and decrease serum IgA. The curative effects of the combined therapy overmatches using western medicine of control group. (3) Microcircuiation disorders may be the mechanisms of hematuria of IgAN, and it needs further research.
