Objective: To study the factors influencing prognosis of intracranial aneurysm after surgical treatment ,the timing of micosurgery in intracranial aneurysm,the methods of treatment in cerebrovascular spasm (CVS) and the preventions of complications.Methods: The clinical data from 465 patients during 1994,3-2004,1,which including 58 variable devided into 168 dummy variable were analysed by means of the logistic regression analysis model SPSS 12.0. The following factors were evaluated: the age and gender of patient, the Fisher grade of spontaneous arachnoid haemorrhage(SAH),the Glasgow Coma Scale(GCS) and Hunt and Hess grading before treatment ,the diameter of aneurysm, the level of blood glucose and the complications,wether the application of medicine per veins to prevent CVS,the times of the aneurysm ruptures before operation,the timing of operation,the duration of temporary clip of artery in operation and some others.Results: There were 486 intracranial aneurysms in 465 cases, of them 19 patients had multiple aneurysms, 184(3 7.86%) aneurysms were in the posterior communicating artery(PCoA), 135(27.78%) in the anterior communicating artery(AcoA),60(12.35) in the internal carotid artery(ICA), 39 in the middle cerebral artery(MCA) ,29 in the anterior cerebral artery(ACA), 13 in the ophthalmic artery (OA); 25(5.14%) in the vertebral-basilar artery (V-BA). The prognosis was related to Hunt and Hess grading before operation, diameter of aneurysm, the value of blood glucose and the complications. While it was not related to the age and gender of patient,the Fisher grade of spontaneous arachnoid haemorrhage(SAH),the Glasgow Coma Scale(GCS) grading before treatment ,wether the application of medicine per veins to prevent CVS,the times of the aneurysm ruptures before operation,the timing of operation,the duration of temporary clip of artery in operation. Theoverall incranial aneurysm mortality was 8.11% at 3 month ,74.9% had a favourable outcome and 16.99% were severely disabled.Conclusions:The factors influencing prognosis of incranial aneurysm include Hunt and Hess grading before operation, the diameters of aneurysms, the level of blood glucose and the complications happened during the treatment course .The clinical prognosis of the intracranial aneurysm patient can be improved by thoroughly evaluating the factors.
