Objective: To investigate the mechanisim of malnutrition in chronic renal failure (CRF) and the adjusting mechanism by the Reinforcing Kidney and Exhausting Toxic Mixture(REM).Methods: 60 wistar rat was divided into 3 groups randomly :the normal contral group(group I ),CRF group(group II ),and CRF rat perfusing with REM group(group III). Taking their fat?kidney?brain tissue for detecting the expression of the leptin? leptin receptor(Ob-R),and neuropeptide Y (NPY) by the means of in situ hybridization and immunohistochemistry.Then,to understand the mechanism that REM affect on CRF patient with malnutrition. ResuIt:Comparing with control group, the leptin while the protein exprese intensely In CRF rat ;In brain tissue ,the expression of Ob-R enhanced and NPY were inhabited; In kidney tissue,the ob-R exprese weakly. After perfusing the REM , comparing with CRF group the Ob-R expression level were decended ,while NPY were reinforced in brain; renal ob-R exprese strongly than CRF group. Conclusion:The CRF are resulted from spleen and kidney asthenia,Blocking of tri-energizer,Tetention of damp and stasis of blood; turbid pathogens in the body. Then,the primary treatment method was reinforcing spleen and kidney, mediate organismal function and exhauste toxin. There are metabolic disorder in leptin-neuroendocrine axis in CRF rats and it is related with nutrient state .Maybe the REM could do a little better with the malnutrition of CRF rats by adjustting the activity of ob-R in brain.
