Objective: Quantificatonally supervise peripheral blood CEAmRNA of colorectal carcinoma patients before and after chemotherapy. To discuss the relationship between the numerical value of CEAmRNA and clinical curative effect and evaluate its clinical significance. Methods: Apply fluorescence quantitative polymerase chain reaction (FQ-PCR) to quantificationally examine and analyze peripheral blood CEAmRNA of colorectal carcinoma patients. The experimental group was 49 cases colorectal carcinoma patients. There were 28 cases Duke′s D stage patients had definitely tumor focus remanet and 21 cases Duke′s C stage after radical surgery. The control group was 18 cases healthy volunteers. The colorectal carcinoma patients had been supervised peripheral blood CEAmRNA before and after three cycles chemotherapy but the healthy volunteers examined only one time. The chemotherapy scheme was improved FOLFOX and estimated clinical curative effect after three cycles chemotherapy. The Duke′s D stage patients were estimated PR,SD or PD according to the change of measurable focus and quantificationally analyzed the relationship between numerical value of CEAmRNA and clinical curative effect. The Duke′s C stage patients were analyzed the change of peripheral blood CEAmRNA before and after three cycles chemotherapy and the trend of change. Results: (1). The average of the quantitative change of the fluorescence quantitative data around chemotherapy was 2.5108 with the trend to decline in the Duke′s D stage patients whose clinical curative effect evaluation was partial response (PR), the trend showed little change in stable disease (SD) patients whose average of the quantitative change was 0.1476, but the trend increased with the average of the quantitative change was –1.8424 in progressive disease (PD) patients. With the expression of peripheral blood CEAmRNA changed, the clinical curative effect is different (F=85.30,P<0.01). (2). The numerical value of peripheral blood CEAmRNA of the Duke′s C stage patients before and after three cycles chemotherapy was changed (t=4.48, p<0.01), the mean value was 1.5138, the expression of CEAmRNA was decline after chemotherapy. (3). There were no peripheral blood CEAmRNA expression in 18 cases healthy volunteers but colorectal carcinoma patients had positive rate was 73.5%. The positive rate of experimental group was markedly higher than control group(χ2=28.528 P<0.01). (4). The peripheral blood CEAmRNA positive rate of Duke′s C staging patients was 57.1%,but Duke′s D stage was 85.7%. The positive rate increased with the disease evolving. The positive expression rate was statistically significant in these two groups(χ2=5.026,P<0.05). Conclusion: (1). The change of numerical value of CEAmRNA of Duke′s D stage patients before and after chemotherapy was significant related to clinical curative effect. (2). The peripheral blood CEAmRNA of the Duke′s C stage patients was still expressing after radical surgery and droped after chemotherapy. (3). There was a close relation between the positive rate of CEAmRNA in the peripheral blood and Duke′s stage. The numerical value of peripheral blood CEAmRNA of colorectal carcinoma patients possibly become a importance index to estimate clinical curative effect and direct clinical treatment.
