Purpose: 1. To determine criterion plane of fallopian and its artery in transvaginal sonography by means of autopsy and transvaginal sonosalpingography. 2. To investigate the hemodynamic changes in fallopian tubal artery (FTA) of women of childbearing age and their correlation with serum levels of ovarian hormones.Material and methods: 1. The pelvises of three female corpses are chosen for anatomy, with one sagittally and the other two coronally anatomized, to observe the distribution of blood vessels, the length, the artery origin and venous circumfluence of every fallopian tube segment. 2. Four 27~31-year old women with a 2~4 years' history of infertility are examined by transvaginal sonosalpingography to investigate the location, shape and characters of fallopian tube and FTA spectral waveform according to the distribution and motion of liquor in fallopian tubes. 3. Twenty-one married women of childbearing age, with regular menstrual cycles, without gynecological diseases and without having any hormonal drug in the last half year, are given serial transvaginal sonography in pre-follicular phase, ovulatory phase, corpus luteal phase and atrophy luteal phase, respectively, to observe images of fallopian tube and FTA. The hemodynamic parameters are also measured. 4. The serum levels of estradiol and progesterone of five women under investigation are measured by fluoroimmunoassay. 5. Data are presented as mean ± S.E.M. One–way analysis of variance (ANOVA) is used to evaluate the data of four phases. The comparison of parametric paired data is made by paired t-test. Pearson correlation coefficient is also calculated to assess correlation between serum hormonal values and blood flow parameters of FTA. P<0.05 is considered statistically significant.Results: 1.The inspected method of fallopian and FTA in transvaginal sonography is transverse scanning in vagina, with the beam of the examined side; the criterion plane is different according to the location of uterus; and the sonographic character is tubal hypoechoic strip adjacent to cornua uteri. 2. Visualization rate of fallopian tube and its artery by transvaginal sonography is 100%. 3. PSV, EDV and TAMAX of tubal artery are relatively low in pre-follicular phase, then rise in ovulatory phase, and reach to peak velocity in corpus luteal phase and fall again in atrophy luteal phase. PI and RI are at the highest level in pre-follicular phase, then decrease in ovulatory phase, and reach the lowest level in corpus luteal phases and begin to rise again in atrophy luteal phase. 4. EDV, TAMAX, PI and RI in four phases of the functional side are found to be significantly different, while no difference is shown in PSV. 5. PSV, TAMAX, PI, RI in four phases of non-functional side are not statistically different, while EDV and RI show significantly different. 6. EDV of functional side is significantly different from that of non-functional side in corpus luteal phase and atrophy luteal phase (P<0.05); TAMAX of the two sides show significantly different in corpus luteal phase (P<0.05); PI and RI of the two sides are found to be statistically different in ovulatory phases (P<0.05, P<0.001), corpus luteal phase (P=0.01, P<0.001) and atrophy luteal phase (P<0.01, P<0.01). 7. RI of functional side correlates positively with the serum level of estrodiol (r=0.45, P<0.05). PSV, EDV, TAMAX, RI of functional side correlate positively with the serum level of progesterone(r= 0.523, P<0.05; r=0.722, P<0.01; r=0.478, P<0.05; r=0.446, P<0.05, respectively).Conclusion: 1. Transvaginal sonography is a convenient and efficient method to examine fallopian tube and its arterial hemodynamics. The inspected method is transverse scanning in vagina, with the beam of the examined side; the criterion plane is different according to the location of uterus; and the sonographic character is tubal hypoechoic strip adjacent to cornua uteri.2.
