Objective: Hepatitis B is a infectitious desease which is a severe threat to human health.In China, the total infection rate of Hepatitis B Virus (HBV) has reached 60%, and more than 1 billion people are chronic HBV carriers.HBV infection can lead to multiful acute and chronic hepatitis, from non-symptom infection to serious progressive hepatitis.Due to complexity of the relationship between HBV and their hosts, the study of hepatitis pathogenesis has not made any breakthrough despite great efforts in basic and clinical medicine.At present, the study of HBV pathogenesis focuses on two aspects, that is, exploring the pathogenesis of eliminating viruses by human bodies, and finding the expression of HBV gene and their infection factors.The slow and constant HBV infection has distinct connection with the identification, responsion and endurance of HBV antigen by human immune systems.Accordingly, immune responsion of HBV diathesis has great significance on the course, pathology and recovery of chronic HBV carriers.The necessity of finding of the factors which lead to a series of abnormity of body immune system, which further results in the constant non-responsion and slowness of HBV infesction, urges us to make a comprehensive study of the immune control by the chronic HBV carriers, in order to explore the mechanism of chronic HBV and establish more effective Rx.The thesis aims at discussing the mechanism of slowness of HBV infection, from the aspect of the control of immune system by body incretionary system, through the study of the changes of the serous level of cortisol and immunological function of lymphocyte of chronic hepatitis B virus carriers.Methods: We collected 60 cases chronic hepatitis B virus carriers.among them36 cases were HBeAg positive , 24 cases HBeAg negative,there were 10 cases normal controls .The serous levels of cortisol were measured by radioimmunoassay,T lymphocyte subsets were detected by Flow cytometry(FCM).Results: compared with those in normal controls, the serous levels of cortisol in chronic hepatitis B virus carriers were singnificantly higher , there is no distinct difference in the proportion of C D4+ T lymphocyte (P>0.05) , but the odds of C D 4+/VC D8+ obviously lower (P<0.01) , the proportion of C D 8+ T cell singnificantly higher (P<0.01). In comparison between HBeAg positive group and HBeAg negative group, the serous levels of cortisol of the former group were singnificantly higher (P<0.05 ), and the proportion of C D 8+ T cell higher too (P<0.05 ). However, there is no distinct difference in the proportion of C D4+ T lymphocyte (P>0.05) .Conclusion: Our results suggested that in chronic hepatitis B virus carriers, the serous levels of cortisol were singnificantly higher than normal, and the immunological function of lymphocyte were out of proportion, there were might be some direct or indirect causality between them,which would has some helpful significance in researching on the mechanism of immune tolerance state and clinic treament of chronic hepatitis B.
