AIMTo explore the changes of peripheral blood CD5+ B cells , traditional B cells,and total B cells among chronic hepatitis B(CHB) patients and chronic a-symptomatic HBV carriers (AsC).METHODSThe percentages of CD5+ B cells ( CD5+ CD19 + ) , traditional B cells( CD5-CD19 + ) , total T cells ( CD5+ ) , and total B cells ( CD19+ ) in 21 patients with chronic hepatitis B and 12 chronic asymptomatic HBV carriers and 10 healthy controls were determined using immunofluorescence double-staining and flow cy-tometry.RESULTSThere were no differences in percentages of CD5+ B cells between healthy controls and chronic asymptomatic HBV carriers. As compared with healthy controls and AsC, the percentages of CD5+ B cells and total B cells increased significantly in chronic hepatitis B patients. There were no differences in the percentages of traditional B cells and total T cells among CHB, AsC and healthy controls.DiscussionThe B cell in peripheral lymph organ is hertrogenietic. The B cell can be divided into rent subsets according to origin, immuno-function, surface signs. The B cell can be divided into B-l and B-2 cell according to expression of CD5. B-2 lymphocyte (traditional B cell) is an important cell in the immuno-system, which induces humoral immunity, and is called B cell.B-l cell is called CD5 + B cell due to its expression of CD5 molecule firstly. The CD5 gene was cloned in 1986, its coded production is a pair of glucose protein with molecule weight of 67000D. The CD5+ B cell is an independent B cell subset which is associated with self-immunological diseases. CD 19 molecule can be found in various stages of B cell growth except plasma cell, which is an important mark of B cell. Recently, aboard research showed that peripheral blood CD5 + B cell increased significantly in patients with HCV infected and was associated with certain self-anti-body production. Hepatitis B is a immunity related disease. There is no direct damage to liver cells after HBV infection, its a indirect damage through inducing immunological reactions. It's worthy to explore whether the CD5+ B cell have been involved in the development of chronic hepatitis B as well as association between the percentages of traditional B cells, total B cells and total T cells and chronic HBV infection.The results showed that the peripheral CD5+ B cell present is 2. 2% in healthy controls which is accordance with other findings. The percentage of CD5 + B cell increased significantly in patients with chronic hepatitis B compared with healthy controls and chronic asymptomatic HBV carriers. There were no differences in percentages of CD5+ B cells between healthy controls and chronic asymptomatic HBV carriers. This finding suggested CD5+ B cells might be associated with CHB. The results did not show an increase in the percentage of B-2 cell (CD5 CD19+), which secreting anti-HBs and cytokine. There were no differences in the percentages of traditional B cells among CHB, AsC and healthy controls. The previous studies showed no anti-HBs- secreting B cells in peripheral blood between patients with chronic HBV infection and healthy con-trols, in the other hand, there were a number of anti-HBs- secreting B cells before anti-HBs present in peripheral blood in acute hepatitis B (AHB) patients. There was no difference in B2 cell with production of anti-body between the heathy controls and patients with chronic HBV infection. Because of the expression of CD5 molecule in total T cells and a small part of B cells ( B1 subset) , some researchers found that no statistical significance in the percentages of T cell among CHB, AsC and controls using CD5 + to represent total T cells. These findings indicated that there were a number of activated CD5+ B cells in patients with CHB.ConclusionThere were a number of activated CD5 + B cells in patients with CHB. The CD5+ B cell might be associated with CHB. There was no difference in traditional B cell among CHB, AcS and healthy contols.
