Objective To explore the safety of misoprostol to fetuses and newborns in induced term labor.Methods 194 eligible primiparas at full pregnancy were randomly divided into three groups:67 subjects were vaginally administered with maximum of 25ug a time , 62 subjects were vaginally administered with maximum of 50ug a time till onset of labor,the another is 65cases ,oxytocin 2.5u was added to 500ml of 5% glucose solution and injected intravenously ,starting at 8 drops per minute and adjusting the drops of oxytocin to the apearance of regular contraction. And artificial puncture of membranes was performed when uterine contraction became active. Umbilical vein blood was taken before the first breath of neonate and endothelin(ET-l) concentrations were measured with ELISA nitric oxide(NO) levels were detemined with nitrite reductase assay.Results There was statistical significant in the success rate of labor induction among the misoprostol 25ug-groups,50ug-groups and the oxytocin groups (p<0.05).but it is no differences between the 25ug-groups and 50ug-groups .There was great difference in amniotic fluid contamination ,meconium,pricipitate dilivery, uterine hypercontractility among the misoprostol 25ug-groups,50ug-groups and the oxytocin groups (p<0.01);and that of the 50ug-groups was higher than that of the 25ug-groups(p<0.05). No differences were noted among the three groups in terms of method of delivery, fetal distress, total stage of labor and Apgar scores (p>0.05) . Concentration of ET-1 and NO were comparable in the study (p>0.05 ) ,so as between the group of amniotic fluid contamination and the group of no (p>0.05) .but there was great difference in between the group of fetal distress and the group of no(p<0.01), There was no ralationship between the total dose of misoprostol and ET-l(r=0.02,p>0.05);so as NO (r=-0.009,P>0.05)Conclution 25ug misoprostol given vaginally every 4 hours,was safe in induced labor for term pregnancy, and well in abortion effect with little side effect.
