Induction The chronic renal failure(CRF) was one of high incidence diseases. Atpresent, kidney transplant was the best method to treatment CRF, but theresource of kidney was very limited. The overwhelming majority patientssustain lives with the long-term hemodialysis and peritoneal dialysis. Thecomplication of heart and brain systems was one of main reasons which thenephrosis patients were died of. The long-term dialysis patients had the serious disorder of lipidmetabolism, this was also one of the dangerous factors which inducedatherosclerosis and cardiovascular diseases. Controlling these patients lipidmetabolism maybe prevent atherosclerosis occurs , then eliminate thecomplication of heart and brain system . This was one of importantresearch domains which domestic and foreign scholars were payingattention to. In recent years, the relationship of lipoprotein(a) levels and thechronic renal failure was noticed. Studies of the association betweenatherosclerosis and Lp(a), the serum Lp(a) levels was related topentanucleotide repeats polymorphism in 5'portion and flanking regionof Apolipoprotein(a) gene. Studies indicated, the chronic renal failure may cause theinflammation reaction and induce the inflammation condition. The earlyresearch displayed that the micro-inflammational cytokines increased in thelong-term hemodialysis patients . In another word , the micro-inflammation was the most important reason in dialysis. But subsequentresearch of Pereira indicated that inflammational cytokines (includingIL-1, IL-6, the TNF-alpha et al.) levels remarkably increased in patientswith chronic renal failure . The purpose of this study was to discuss the association of Lp(a)levels in hemodialysis patients with Apo(a)PNR polymorphism ,inflammational cytokines(IL-1β, IL-6, L-8, TNF-α) and the marker ofoxidative stress (ox-LDL), and to clarify the mechanism of Lp(a) inhemodialysis patients. Method The study was conducted on a sample of 53 patients (males: 37,females: 16, mean age between 23~75 years old) confirmed chronic renalfailure and carried on the hemodialysis treatment above one year. 42samples in healthy physical examinations (male: 28, Female: 24, mean agebetween 24~69 years old),who were proved to be healthy without brain,kidney, liver and so on relevant disease at usual physical examination, wereused as controls. Apo(a)5' PNR polymorphism gene fragment wasamplified by PCR. The productions were examined by microintegratedcapillary electrophoresis chips. End-point method examined TC , TG , theturbidimetric method examined Lp(a), HDL-C, LDL-C, ApoA and ApoB inthe AEROSET-2000 biochemistry autoanalyzer. Ox-LDL, IL-1 beta, IL-6,IL-8 and TNF-alpha quantification was performed from frozen plasma withdouble-antibody ELISA. Result Serum Lp(a) levels remarkably was higher in patients with chronicrenal failure than control subject (P<0.05), TC, LDL-C, ApoB levelsremarkably was higher than control(P<0.05). The HDL-C level was lowerthan control , but two groups' had no the significant difference . TG andApoA concentration difference had no significance. There was notcorrelation between Lp(a) and other lipids. The tandem frequency ofcrowds Apo (a) PNR in chronic renal failure and control had 6 alleles of 4,5, 7, 8, 9, 10 and 12 genotypes of 4/9, 5/8, 5/9, 7/7, 7/8, 7/9, 7/10, 8/8, 8/9 ,8/10 , 9/9 , 9/10 ; the frequency of 8 alleles was remarkably higher thanothers . Two groups' Apo(a) the PNR genotype and the allele frequency hadthe remarkable difference , but genotype distributions between males andfemales were not remarkable difference. In hemodialysis patients serumox-LDL concentration was remarkably higher than controls (P<0.01). Inthe hemodialysis group , IL-6, IL-8, the TNF- alpha levels were remarkablyhigher than the control subjects ,and the IL-1 beta level had no significantedifference. There was significante correlation between Lp(a) and IL-6,IL-8. Conclusion Lp(a) levels related to Apo(a) (TTTTA)n repeats number in thehemodailysis patients .But serum Lp(a) level was related to Apo(a)(TTTTA)n polymor...
