Objective: To investigate the difference of ankle brachial index (ABI) and toe brachial index (TBI) in patients with type 2 diabetes, and evaluate the significance of ABI and TBI in judging lower extremity ischemia. Methods: ABI and TBI were measured by personal vascular lab on 142 subjects with type 2 diabetes and 40 control subjects. The history of smoking,diadetes duration,insulin therapy,hypertension,dyslipidemia were recorded.The 142 type 2 diabetic subjects were divided into varied groups according to the results. Result: TBI in type 2 diabetic subjects were lower than those in control subjects(0.71vs0.80,P<0.01), ABI between the control subjects and type 2 diabetic subjects without lower extremity arteral disease(LEAD) had no significant difference.ABI and TBI between with and without hypertension, dyslipidemia, smoking were different (P<0.01). Between ABI<1.3 group and ABI≥1.3 group had no differences in TBI. The correlation coefficient of ABI and TBI in control subjects was 0.44(P<0.01), in type 2 diabetic subjects with lower extremity arterial disease was 0.32(P<0.01). Kappa statistic was used to assess the agreement of ABI and TBI,the Kappa was 0.34(P<0.05) . ABI-TBI had no significant difference between control subjects and ABI<1.3 group,but both 1.15
