Background Male infertility is an important research area of Reproduction. According to reported documents, 10% couples experience infertility, and male factors are about 40%. The etiological factors of male infertility are complicated, including endocrine factors, immune factors, biological factors, genetic factors, environmental factors and so on. Many etiological factors which cause male infertility effect mainly on seminal and sperm parameters. So the abnormal of the seminal and sperm parameters are the real causes of the male infertility. Male infertility caused by infection is increasing. Infection could destroy the blood-testis-barrier and make antigen of sperm expose. Then the antibody against sperms will appear in blood and seminal plasma, and this will cause immunoinfertility. In recent years, the assay methods of seminal and sperm parameters have been updated with the advance of computer technology. Computer assisted sperm analysis (CASA), sperm motion parameters analysis and acrosomal enzyme activity test are most popular and important assay methods. With the computer assisting, we can get seminal and sperm parameters more precisely and objectively. Objective With the computer assisting, we will discuss the influence of infection and immune factors on seminal and sperm parameters, and try to explain the mechanism of these factors on male infertility. Method From 2002 to 2005, We have collected 2367 seminal specimens of male infertility, and classified these specimens according to abnormal factors. Then we have analyzed seminal parameters, sperm motion parameters, acrosomal enzyme activity of every case. Using statistic method, we try to find out relationships of seminal and sperm parameters with WBC in semen, UU infected and seminal plasma AsAb. Result 1. In the 2367 cases of male infertility: the cases that abnormal WBC increasing in semen are 43.50%, the cases of UU infected are 44.28%, the cases of positive seminal plasma AsAb are 8.90%. 2. In the group of WBC>5 /HP: the density of sperms, motility rate of sperms, a,b grade sperms are significantly lower than that of WBC≤5/HP group (P<0.001), but teratosperm rate and consistency of semen are significantly higher than that of WBC≤5/HP group (P<0.001). In the group of WBC>5 /HP: the percentage of morphologically abnormal sperms are significantly higher than the control group (P<0.05), including: both small head sperms and tapered sperms are significantly higher than that of WBC≤5/HP group (P<0.05). Motion parameters of sperms: In the group of WBC>5 /HP: VCL, VAP, VSL are significantly lower than those of WBC≤5 /HP group (P<0.001), MAD, BCF are significantly hihger than the group of WBC≤5 /HP(P<0.001). VCL, VAP, VSL are significantly negative correlated with WBC in semen (P<0.001) and MAD, BCF are positive correlated significantly with WBC in semen (P<0.001). there are no correlationships between ALH, LIN, WOB, STR and WBC in semen. In the group of WBC > 5 /HP: total acrosomal enzyme activity is 14.01±10.22 (μIU/106sperm), in the group of WBC≤5/HP, total acrosomal enzyme activity is 15.98±10.70(μIU/106sperm), there is significantly differencebetween two groups (P<0.05). In the group of WBC>5/HP: the rate of acrosome intactness is 85.38±10.24 (%), it is 88.06±8.60 (%) in group WBC≤5/HP, there is significant difference between two group (P<0.05). 3. In UU (+) group: Density of sperms, motility rate of sperms, a, b grade sperms are significant lower than those of UU group (-) (P<0.001, P<0.05, P<0.01). In UU (+) group: the rate of teratosperm and the liquefied time of semen are significant higher than that of UU (-) group (P<0.05, P<0.001). Besides In the group of heightened consistency of semen: the positive rate of UU is significantly higher than that of the normal consistency group (P<0.001). Motion parameters of sperm: VCL, VAP, VSL of UU (+) group are significantly lower than those of UU (-) group(P<0.001). MAD, BCF of UU (+) group are obviously higher than those of UU (-) group (P<0.05). there are no significant differences at ALP, LIN, WOB, STR of two groups. In UU (+) group: total acrosomal enzyme activity is 15.32 ±9.79 (μIU/106sperm), in UU (-) group, total acrosomal enzyme activity is 16.79±9.92 (μIU/106sperm), there is significant difference between two groups. UU could decrease acrosomal enzyme activity 4. In seminal plasma AsAb (+) group: the volume of semen, density of sperms, motility rate of sperms, a, b grade sperms are significantly lower than that of AsAb (-) group (P<0.001). In seminal plasma AsAb (+) group: the liquefied time of semen is significantly higher than that of AsAb (-) group (P<0.001). In the group of heightened consistency of semen: the positive rate of seminal plasma AsAb is higher significantly than group of normal consistency of semen (P<0.001). Sperm motion parameters: VCL, VSL, VAP, WOB,STR are significant lower in AsAb (+) group than those of AsAb (-) group (P<0.001, P<0.05). MAD, ALH,BCF, LIN are not significantly different between two groups. In seminal plasma AsAb (+) group: total acrosomal enzyme activity is 15.65±11.48 (μIU/106sperm), it is 15.73±10.74 (μIU/106sperm) of AsAb(-) group. There is no significant difference between two groups in total acrosomal enzyme activity. 5. Compare WBC>5/HP group with WBC≤5/HP group, the positive rate of UU and seminal plasma AsAb are significantly higher in group of WBC>5 /HP (P<0.001). Compare UU (+) group with UU (-) group: the positive rate of seminal plasma AsAb is significantly higher in group of UU (+)than UU (-) group (P<0.001). Conclusion: 1. According to analysis of seminal specimens, we found that the cases of Abnormal increasing WBC in semen, UU infection of semen are important and large portion of male infertility. They can make the positive rate of AsAb in seminal plasma more high. 2. Abnormal increasing WBC in semen can make density of sperms, motility rate of sperms, a,b grade sperms lower than that of control group respectively. But it make consistency of semen and the percentage of abnormal sperm morphologically is significantly higher than that of control group. Abnormal increasing WBC in semen has great impact on sperm motion parameters. Especially, VCL, VSL, VAP decrease significantly and they are negative correlate with WBC in semen. Abnormal increasing WBC in semen also make total acrosomal enzyme activity and the rate of acrosome intactness significantly lower than those of control group. 3. Infection of UU could impact seminal parameters obviously. Density of sperms, motility rate of sperms, a, b grade sperms are lower than those of control...
