Objective: To discuss microsurgical treatment to petrosal-clivus meningioma and crucial points in operation, and introduce the microsurgical experience of improving the rate of total removal in petrosal-clivus meningioma with minimal morbidity. Method: A series of 34 cases operated in our department was analysed respectively. There were 14 males, 20 females, with age between 26~67, mean age 45.6. CT and MRI were performed in supporting preoperative diagnosis and tumor localization, all patients were treated with microsurgical techniques and histologically verified meningiomas. The presenting features, operative details and complications were documented. Results: The modified subtemporal-occipital approach was commonly used in most cases. Total resection of the tumor was achieved in 26 of 34 patients, subtotal in5, and most partial in 3, two patients died. Surgical effects were satisfying. Conclusions: Petrosal-clivus meningioma represents a difficult surgical challenge. Contemporary microsurgical techniques play an important role in the treatment of petrosal-clivus meningioma. The preoperative diagnosis results obtained by CT and MRI scan are helpful in preoperative appraisal.
