An Clinic Study Of Using Pravastatin To Prevent The Steroid-induced Femoral Head Necrosis

Object : To investigate whether the Pravastatine can prevent the course of result in Steroid-induced Femoral Head Necrosis (SIFHN). Methods: The patients who needed therapy by steroid, 20~46 years old ,were randomly divided into 2 groups, Group contrast (n=20): treated by steroid single. Group therapy (n=20): treated by steroid and pravastatin 10mg·d-1 p.o. All patients were evaluated with clinic findings of hip joints,serum lipid,pelvis X-ray photograph and two hip joints MRI in prep-treated,6 months and 12 months of post-treated. Result: In the Group contrast, TC,TG and LDL-C of the patients were increased significantly in 6 months and 12 months of post-treated, higher than those in prep-treated significantly(P<0.05); there were 6 patients and 9 patients who had MRI change of femoral head necrosis in 6 months and 12 months of post-treated. In the Group therapy, TC,TG and LDL-C of the patients were not increased significantly in 6 months and 12 months of post-treated, not higher than those in prep-treated (P>0.05),but lower than those in the Group contrast(P<0.01); the patients who had MRI change of femoral head necrosis in 6 months and 12 months of post-treated were lower than who in the Group contrast(P<0.05). The number and the total number of the patients who had MRI change of femoral head necrosis was higher than the number and the total number of the patients who had X-ray change of femoral head necrosis in 6 months and 12 months of post-treated in the Group contrast (P<0.05) and in the two Groud(P<0.05). Conclusion: Pravastatin can prevent the occurrence of SIFHN effectively by lowering the level of serum lipid; MRI can show the pathological change of SIFHN exactly and early, its sensitivity and value of diagnosis are higher than X-ray.
