objective:To study clinical significance and state of activated blood platelets in patients with mild and severe acute pancreatitis. Methods:Clinical data: 26 cases with acute pancreatitis, according to diagnostic standards made by Chinese Medical Association in 1996, from October 2003 and February 2005 in our hospital were chosen and divided into 3 groups: severe acute pancreatitis group, 13 cases(5 males,8 females,aged 31-65 years); mild acute pancreatitis group, 13 cases(3 males, 10females,aged 35-63 years); normal subject group, 20 cases(10 males, 10 females,aged 28-65 years, all of them are healthy persons of the medical examination). All patients were applied the standarded medical treatment. Every groups were compared in age and gender.By using flow cytometry and platelet specific monoclonal antibodies, the CD62p and PAC-1 levels were dynamically observed in 13 cases of severe acute pancreatitis, 13 cases of mild acute pancreatitis and 20 normal subjects.Statistical method: The data were figured by mean standard deviation. We applied the SPSS 10. 0 to conduct data analysis by using the analyzing of One-Way ANOVA and repeated measure ANOVA.Results:1. The significant difference in the CD62p and PAC-1 levels was found between severe acute pancreatitis group, mild acute pancreatitis group and normal subject group before treatment (P<0.01) ; the CD62p and PAC-1 levels in severe acute pancreatitis group was significantly higher than the two groups (P0.05) .4. After effectively treating the levels of CD62p and PAC-1 in severe acute pancreatitis group were significantly decreased; especially the levels of PAC-1 presented the more downward trend. Interaction was existed in the two indexes.Conclusion:1. The blood platelets in patients with mild acute pancreatitis were not obviously activated before and after treatment. The cause may be that early beginning factor can not activate or completely activate a great volume of inflammatory mediators in these patients. In addition microcirculation disorder was light. In the end "cascade" was blocked during its pathologic damnification.2. Highly activated blood platelets in severe acute pancreatitis may be a important key role that there is microcirculation disorder in its pathologic damnification. Activated blood platelets take part in this physiological andpathological damnification and deteriorate it.3. The levels of CD62p and PAC-1 of bloody plasma can respond accurately the state of blood platelets. Dynamically observing them may get to judgement , inspectabi-lity and forecast of disease and there will be a good clinical significance.4. At the early stage of acute pancreatitis activated blood platelets in patients will be antagonized to ameliorate microcirculation disorder. This will become a way which can cure acute pancreatitis.
