| Background In the past several years , many epidemiological and clinical researches show that high pulse pressure(PP) correlate with lager artery atherosclerosis and small blood vessel damage in middle and old hypertensive patients. PP has become stronger predictive factors for organ damage. Objective To investigate the correlation of pulse pressure with structure and function of cardiovascular, blood vessel and kidney in the essential hypertensions . Methods 153 essential hypertensive patients were measured, in their left ventricular mass index (LVMI),ejection fraction(EF),the ratio between early diastolic peak flow velocity and atrium peak flow velocity(E/A),internal medial thickness(IMT)of carotid artery, microalbumiuria(MAU) and carotid-femoral pulse wave velocity (C-FPWV). All subjects were divided into two groups by PP level (PP<60mmHg or PP≥60mmHg) ; According to age, all subjects were divided into four subgroups ,as follow:〈50 years ,50-60 years ,60-70 years ,〉70 years; According to LVMI>125g/m~2 (male),LVMI〉120 g/m~2(female), EF<50%, MAU>50mg/L, IMT≥1.3mm , C-FPWV>10.0m/s, all subjects were divided into two subgroups corresponsively. Results t-text showed that LVMI, EF, E/A, MAU, IMT, PWV have statistical significance between PP〈60mmHg and PP≥60mmHg group. In every age group, the difference still existed. Meanwhile, the patients with subclinical target organ damages have higher SBP and PP. Pearson analysis showed that LMVI, MAU, IMT ,PWV had positive corrections with age and PP, and EF,E/A had negative correction with PP . Furthermore ,by multiple factors stepwise regressive analysis we still found that LVMI=0.356PP-0.936BMI+92.39 , r2=0.188; EF=-0.0008PP-0.0012age+0.713,r2=0.220; EA=-0.003PP+1.11, r2=0.076; IMT=0.006PP-0.006DBP+1.31, r2=0.153; MAU=0.651PP+1.11BMI-36.64, r2=0.589。C-FPWV=0.057PP+0.033height,r2=0.315. Conclusion PP increases with age; The essential hypertensions with high PP had more apparently damaged target organ than those without high PP. PP is a stronger predictive factors to subclinical organ damages . |
