BACKGROUDEndometriosis(EMS) is a common gynecologic disease. EMS is a benign disease, but it can proliferate, infiltrate and recur.Chronic pelvic pain and infertility deeply influence woman s health and living quality. The etiology of EMS is not clear to this day. Through investigating intercellular adhesion molecule-1 (ICAM-1) which plays an important role in adhesion, we hope to gain some progress on etiology of EMS, and provide a credible key for cure and estimate recurrence.OBJECTIVEStudy the expression of ICAM-1 in eutopic and ectopic endometrium, to investigate the difference of ICAM-1 in eutopic endometrium between cases and controls, and the relation between ICAM-1 and stage. Study the expression of sICAM-1 in serum, peritoneal fluid and urine, to investigate the relationship among intercellular adhesion molecule-1 (sICAM-l),stage and estimate recurrence, find the theory that sICAM-1 can be cure index, standard scope, and a new target to cure EMS.METHODS AND RESULTS1.The expression of ICAM-1 in eutopic and ectopic endometrium of patients with EMS:1.1 The expression of ICAM-1 were detected by using immunohistochemical method in 36 specimens of endometrium and 36 specimens of endometriotic tissue from patients with EMS and 30 specimens of endometrium from patients without EMS. Results:The expression of ICAM-1 among the three groups, endometriotic tissue > endometrium from non-EMS > endometrium from EMS.1.2 The expression of ICAM-1 protein were determined by immunoblotting method in endometrium with 34 patients with EMS (16 in follicular phase and 18 in luteal phase) and 32 non-EMS subjects (12 in follicular phase and 20 in luteal phase). The results were scanned and semi-quantified. Results:?The molecular weight range of ICAM-1 is 97~116KD.?ICAM-1 expression on endometrium in menstrual cycle showed the same trend, P=0.08.?Endometrium from patients with EMS expressed significantly fewer ICAM-1 protein than those without EMS, P=0.013.2.The expression of sICAM-1 in body fluid of EMS:2.1 The level of sICAM-1 in the serum: 76 after-surgical patients with EMS (32 in follicular phase and 44 in luteal phase) and 44 after-surgical patients without EMS (20 in follicular phase and 24 in luteal phase) were studied by ELISA. Results:?There was no significant difference between the two groups, P=0.13.?No difference was found between proliferation and secretary phase in women with and without EMS, P=0.35.2.2 The level of sICAM-1 in the peritoneal fluid: 47 peritoneal fluid of patients with EMS and 41 peritoneal fluid of patients without EMS were studied by ELISA. Results: ? The expression of sICAM-1 in the peritoneal fluid of patients with EMS were significantly higher than the controls.?The results fromI - II phase and III-FV phase patients with EMS showed the same trend, P=0.37.2.3 The level of sICAM-1 in the urine: 40 before-surgical urine (11 in I -II phase,29 in III-IV phase) and 46 after-surgical urine of patients with EMS, 30 before-surgical urine and 49 after-surgical urine without EMS were studied by ELISA. Results:? There was no significant difference between the patients with and without EMS in the before-surgical urine,P=0.5.? There was no significant difference between before-surgical patients and after-surgical patients with EMS,P=0.19.(3)There was no significant difference between the early patients and the late patients with EMS in the urine,P=0.18.CONCLUSIONl.The abnormal expression of ICAM-1 in endometrium from patients with EMS may play a role in EMS, also explained the decisive function the difference of the endometrium played in EMS occurrence, verified the "endometrium decision"theory.2.The influence estrogen and progestogen plays on ICAM-1 is one of the reason of EMS, but ICAM-1 also plays an important role in EMS.3.The obvious difference of sICAM-1 in the peritoneal fluid in EMS and in control, explain the function of sICAM-1 in EMS. But the relation between sICAM-1 in the serum and the urine and EMS needs further research.
