Objective To investigate the association of the polymorphism of eNOS gene T786C and 27bp varible number of tandem repeats with essential hypertension.Methods Two hundred and sixty-six patients with EH and matched controls were selected.Polymerase chain reaction combined with restriction enzyme digestion was used to detect the polymorphism of eNOS gene. Polymorphism of eNOS gene 27bp VNTR were determined by PCR and agarose gel electrophoresis.Questionnaire refered to smoking,drinking, body mass index,etc was administered.Results (1)BMI was significantly higher in patients with EH than that in controls.Logistic regression analysis showed that BMI percentage correlated with the risk of EH.After adjustment for the other risk factors,BMI was still an independent risk factor of EH. (r=0.421, P<0.05 ) .(2)There was no significant difference noticed in genotype distribution for the eNOS gene T786C genotype between hypertensive groups and controls.SBP and DBP of patients with C allele of eNOS gene were higher than those of TT genotype,but the differences were not significant.(3)Positive effect of the T786C mutant genotypes on SBP was significant in the drinkers and men with EH.(4)The frequencies of a allele and ab+aa genotype in EH were significantly higher than in control group. SBP and DBP of patients with a allele of eNOS gene werehigher than those of bb genotype,but the differences were not significant.(5) Positive effect of the 27 bp VNTR mutant genotypes on SBP was significant in the drinkers , men and overweight with EH.(6)There was no relationship between eNOS T786C and 27 bp VNTR.Conclusion (1) BMI was an independent risk factor of EH. (2)Although the polymorphism of eNOS gene T786C did not seem to play an important and direct role in the pathogenesis of EH it might have indirect effects through certain risk factors. (3) The polymorphism of eNOS gene 27 bp VNTR is associated with EH,a allele might be involved in the etiology of EH through certain risk factors. (4) There was no relationship between eNOS T786C and 27 bp VNTR.
