The rapid advance of information and communication technology in the past few years brings new hope for the introduction and popularity of telemedicine. The release of .NET provides powerful tools to develop Web applications. Under these circumstances, it is of great value both in academy and business to develop a telemedicine system based on .NET. And during the development, well-formed medical record structure and a medical record management system is necessary and important.This paper is a description of the work on the design and realization of a Medical Record Management System, which is a part of a telemedicine system based on Microsoft .NET. First, concepts of telemedicine, electronic health record and .NET are given, and features of the Medical Record Management System are provided. Then, analysis is given on components and structure of medical record, both in paper and electronic form, together with introduction to some standards relative to structured medical record, such as XML, HL7 CDA and MML, and explanation for what is needed in a teleconsultation, how they should be organized and realized in a database, and how to deal with consultation data granularity using XML with database. And then the work during the realization stage is expatiated, including system analysis and design, database design, and system realization. At last, the whole process of the system development is summarized, and application of decision support system and expert system in telemedicine is predicted.The contributions of the subject are as follows. First, medical record structure for the telemedicine system is defined and a medical record management system is implemented. Second, during the design and implementation, MML (Medical Markup Language), a standard conformed to and expanding HL7 CDA is referenced, which brings certain level of standardization and structurization to the system. And during implementation, the flexible .NET multi-layer architecture is used, and smart client is applied, which is a technique combining the advantage of both B/S and C/S.
