ObjectiveTo explore the indexes of the micro syndrome differentiation in patients with IgA nephropathy (IgAN). MethodsTo analyze the clinical and pathological data of 61 IgAN patients, among which 36 cases with Deficiency of both vital Qi and Yin syndrome. ResultsIn the 61 IgAN patients, 36 cases were diagnosed with Deficiency of both vital Qi and Yin syndrome (DQY) while 25 cases with non Deficiency of both vital Qi and Yin syndrome (non-DQY). In patients with DQY, mourn and fatigue in lumbus was the most common symptom;most clinical manifestations were continued hematuria under microscopy, often accompanied with mild and midrange proteinuria (<2g/24h), seldom with hypertension and/or renal dysfunction. Compared with those in patients with non-DQY, plasma levels of serum creatine (SCr) , total urinary protein for 24h (Upro) , triglyceride (TG) and total cholesterol (TC) were obviously lower while creatinine clearance rate (CCr) was obviously higher;according to pathological typing of WHO (1982) , II — IV were mostly seen;the DQY was related to the deposition of immunocomplex in the mesangial region , most of which with deposition of IgA(+~++) and only 25% of which companied by the deposition of IgG, IgM and C3;and scores of glomerular index, intercapillary cells accrementition, intercapillary base accrementition and tubular atrophy significantly higher while those of interstitial inflammatory cell infiltration and interstitial fibrosis significantly lower, but vascular index, glomerular sclerosis index and crescent index has no significant contrast.ConclusionDeficiency of both vital Qi and Yin syndrome is the basic syndrome of IgAN. To diagnose it earlier will help TCM to play an important role in the therapy of IgAN. Some clinical and pathological indexes would be the reference material of the diagnosis of micro syndrome differentiation on Deficiency of both vital Qi and Yin syndrome of IgAN.
