The Investigation Of The Relationship Between Damp-heat And Infection In The Primary Nephrotic Syndrome

Objective: To observe the different infection between the damp-heat patients and non-damp-heat patients of PNS, find out the different between before and after treatment of the level of leucocytes, CRP, TNF-α, IL-2, IL-6 in two group. In order to find out the relationship between damp-heat and infection in the way of PNS occurred and developed, the evaluative norm and the effecting way of the damp-heat syndrome of PNS, the therapy, the effecting way and the evaluative norm of the Clearing away heat and promoting diuresis.Methods: It' s a one-blind control clinical research. Sixty patients who were selected by entrance standards were classified into damp-heat group(30 patients) and non-damp-heat group(30 patients).All of the patients of two groups came from the inpatients of the first subsidiary hospital of Chinese medicine university in Guangzhou. Traditional Chinese Medicine and western medicine routinely treated both groups. The patients of damp-heat group were treated with prescription of Clearing away heat and promoting diuresis(including: Shiwei, Cangzhu, Huangbai, Shengyiren, Banlangen, Yuxingcao, Baimaogen, Zihuadiding). To observe the different, characterist of icinfection in the both groups. Before, during and after the treatment, the laboratory markers(WBC, GRAN, GRAN%, CRP, TNF-α, IL-2, IL-6) of two groups were observed. And the changes of the integral of symptoms and physical sign in damp-heat group were observed before and after treatment.Result: The percent of infection cases in the damp-heat group is 76. 67%, higher than non-damp-heat group (43. 33%). Before treatment, the level of blood CRP, TNF-α, IL-2 and IL-6 in the damp-heat group is higher than the non-damp-heat group. This result stills subsist during the treatment. But after treatment, it has no different between two group. In the damp-heat group, the level of blood CRP, TNF-α, IL-2 and IL-6 after treatment is lower than thelevel before or during treatment. In the non-damp-heat group, the level of blood CRP after treatment is lower than the level before or during treatment. In the damp-heat group, the integrals of symptoms and physical sign before treatment are higher than the integrals after or during treatment.Conclusion: There is a relationship between the damp-heat syndrome and infection in the patient of Primary nephritic syndrome, for the nephritic syndrome patient induced by infection has represented obvious damp-heat syndrome. There is a relationship between the immune conduction and the way which the damp-heat' s approach to primary nephritic syndrome. IL-2 and IL-6 can be one of the lab guideline of damp-heat syndrome in primary nephritic syndrome. The Chinese traditional medicines, which can clear away heat and promote diuresis, can mend the symptom of damp-heat syndrome in primary nephritic syndrome. Also they have the function of adjusting immunity, resisting inflaming, resisting infection,etc. TNF-a, IL-2, IL-6 can be one of the method to prove the curative effect of the Chinese traditional medicines which can clear away heat and promote diuresis. Clearing away heat and promoting diuresis is an effecting method to cure primary nephritic syndrome.
