The Clinical Effect On Primary Nephrotic Syndrome With The Therapy Of Clearing Away Heat, Nourishing Yin And Promoting Blood Circulation

ObjectiveBy prospective randomized controlled method, to evaluate the clinical effects and to discuss the mechanism of prevention of Hypercoagulation state on primary nephrotic syndrome with the therapy of nourishing yin, clearing away heat and promoting blood circulation.Methods40 cases of patients with primary nephrotic syndrome were divided into testing group and control group by 3:1 ratio. We gave the same basic treatment for all the patients. At the same time, we gave Modified Zhibai Dihuang decoction to testing group and persantine to control group. The symptomatic scores of TCM, urine protein, Serum albumin, blood lipids, coagulation, liver and kidney function were compared before treatment and after 4 weeks treatment.Results(1) After treatment, The integral calculus of symptom of TCM decreased significantly in testing group. The difference was statistically significant (P<0.01), while in the control group, the scores was not decreased significantly, The difference was not statistically significant (P>0.05).The reduction of scores of testing group was greater than that of control group, the difference was statistically significant (P<0.01). (2) Regarding the clinical effect of the two groups, the total effective rate of testing group was higher than that of control group, while the difference was not statistically significant (P>0.05). (3) Compared before and after treatment, The proteinuria, triglyceride, blood cholesterol, Low-density lipoprotein, platelet and serum fibrinogen were all decreased than that of before treatment in both group, Serum albumin was increased in both group after treatment, The difference was statistically significant (P<0.01), The changes of the testing group were more significantly than that of the control group, the difference was statistically significant (P<0.01). In the testing group, prothrombintime was longer after treatment, the difference was statistically significant (P<0.01).The testing group and control group in liver and kidney function before and after treatment were within the normal range, during and after treatment, the patients in both group didn't have bleeding tendency and allergies.ConclusionThe therapy of clearing away heat, nourishing yin and promoting blood circulation (modified of Zhibai Dihuang Decoction) could better alleviate the syndromes of deficiency of yin and damp-heat, to a certain extent, it could reduce proteinuria, increase the serum albumin, and improve hyperlipidemia and hypercoagulation state, etc., it could improve the clinical efficacy of comprehensive deficiency of yin and dampness-heat in PNS patient. It indicated that the effect of using therapy of the clearing away heat, nourishing yin and promoting blood circulation together with prednisone was better than using persantine together with prednisone.
