To explore the pathogenesis of varicocele , we studied the pathological changes of cremaster of varicocele patients with the non-varicocele patients being the control group. 83 cases of varicocele patients were diagnosed as clinical type through physical examination and B-ultrasonography. They were definited as I ° ,II ° and III° separately according to the extent of pathologic lesion indicated by PE and B-ultrasonography and divided into group A, B and C. Biopsy on the cremaster were performed duing operation for varicocele. Group A: 25 cases, unilateral lesion and biopsy on ipsilateral cremaster; group B: 38 cases, unilateral lesion and biopsy on bilateral cremaster; group C: 20 cases, bilateral lesion and biopsy on bilateral cremaster. Control group: 20 cases of indirect inguinal hernia, with varicocele being excluded through PE and B-ultrasonography. Biopsy on the cremaster were performed duing operation for the indirect inguinal hernia. Totally 151 pathological sections on light microscope and 10 pathological sections on electron microscope were studied. According to the pathological report , the discordant dysplasia of cremaster was observed in most of varicocele cases. Varicocele group: 103 sections: I ° , 24 sections, II° , 37 sections, III° , 42 sections. Normal : 16 sections (16%), dysplasia (mildly and severely) 87 sections (84%) .dysplasia of cremaster was observed in most of varicocele cases. The ratio of severely dysplasia of cremaster in I ° , II° and III° varicocele cases were 13%, 35% and 52% seperately, the more severe of varicocele, the more severe of dysplasia of cremaster.
