The Prevalence Of 4G5G Polymorphism Of Plasminogen Activator Inhibitor-1 (PAI-1) Gene In Polycystic Ovarian Syndrome And Its Association With Plasma PAI-1 Levels

Objective:To investigate the prevalence of 4G5G polymorphism of plasminogen activator inhibitor-1 (PAI-1) gene in polycystic ovary syndrome (PCOS) and its functional significance.Methods:We studied 45 patients and 48 controls. Body mass index (BMI) and waist-to-hip (WHR) ratio were determined. Blood samples were obtained for DNA analysis. PAI-1 plasma levels, serum free testosterone, fasting insulin and fasting glucose were measured by the ELISA in all subjects. The 4G5G polymorphic site of the PAI-1 gene promoter region was amplified by the polymerase chain reaction (PCR).Results: There was a statistically significant difference in the distribution of PAI-1 gene variations among the groups(P<0.05). The PCOS group had significantly higher 4G/4G than that of the control group, whereas 5G/5G were less significantly. Among the PCOS women, the genotype 4G/4G was 46.67%, 4G/5G was 46.67% and 5G/5G was 6.67%. Comparatively, among the control group, the genotype 4G/4G was 22.92%, 4G/5G was 58.33% and 5G/5G was 18.75%. In the 4G/4G genotype subgroup 65.63% were PCOS and 34.38% were controls, in the 4G/5G were 42.86% and 57.14% and in the 5G/5G were 25% and 75% respectively. The group of PCOS had significantly higher PAI-1 levels than that of the controls (30.31±15.88 ng/ml vs 22.14±3.98 ng/ml, P<0.05). In PCOS group, PAI-1 plasma level was 36.16±21.62 ng/ml for 4G/4G genotype, 25.79±4.10 ng/ml for 4G/5G and 20.96±1.40 ng/ml for 5G/5G. In the control group the 4G/4G had 24.80±2.05 ng/ml PAI-1 plasma levels, the 4G/5G 22.26±3.71 ng/ml and the 5G/5G 19.50±4.50 ng/ml.
