The Changes Of Gastric Parietal Cells Density And Functional Status And Clinical Effect In The Patients With Duodenal Ulcer Treated By Rabeprazole

Objective:To explore the changes of gastric parietal cell density and functional status and to observe the clinical effects in the patients with duodenal ulcer after Rabeprazole treatment.Methods:(1)We studied 32 patients who were diagnosed endoscopically with duodenal ulcer and a control group of 24 patients with chronic superficial gastritis .Endoscopic examination of the upper digestive tract was done early in the morning after an overnight fast. Three biopsies were obtained in the mid-gastric corpus (one on the lesser curvature, two on the greater curvature);one were obtained in the antrum 3 cm from the pylorus in the greater curvature.One sample from each area of the gastric corpus was processed for light microscopy,and the other sample from gastric corpus for electron microscopy.The antral biopsy was used for HP culture.(2)HP positive patients with DU received tripple regimens(Rabeprazole plus twoantibiotics) for 1 week,followed by Rabeprazole 20mg once daily for 3 weeks. After the treatment, all the patients received endoscopy again.Results:The parietal cell density in DU group(44.41±4.16 ) was significantly higher than in controls(42.37 ± 4.51) (p<0.05) ,but there was no difference between HP positive (44.35 ± 3.69) and HP negative DU subjects(44.67 ± 6.25) or between HP positive (42.90±4.39) and HP negative gastritis (42.75 ± 4.68) (p>0.05) .Parietal cell density (44.41 ± 4.16) was significantly lower after the Rabeprazole treatment(42.44± 4.72) (p<0.05) ,and the percentage of activated parietal cells were lower too.Rabeprazole could cure DU and rapidly relieve the symptom of abdominal pain,acid reflux ,heartburn.The cure rate ,healing rate,effective rate after 4 weeks treatment were 40.6%,90.6% and 100% respectively.Conculsion: The parietal cell density in DU patients was significantly higher than that of chronic superficial gastritis, and markedly reduced after the Rabeprazole treatment, the percentage of activated parietal cells were declined too.Rabeprazole is effective and safe enough in the treatment of duodenal ulcer.It can rapidly relieve the symptom and obtain a high healing rate of DU.
