| Objective:Through the clinical observation on the treatment of Hp related ulcer(spleen-stomach asthenia cold type)by the combination of xiangsha lizhongtang and rabeprazole,the curative effect and safety were clarified,and the basis was provided for the treatment of integrated traditional Chinese and western medicine.Methods : A total of 148 patients with hp-associated ulcer with spleen-stomach deficiency and cold syndrome were randomly divided into 2 groups,including 74 patients in the treatment group and 74 patients in the control group.On the basis of a 14-day course of treatment to eradicate helicobacter pylori in the two groups,the control group was given rabeprazole in sequence,and the treatment group was given rabeprazole plus consol medium decoction.The quality of ulcer healing,the rate of Hp eradication and the curative effect of TCM symptoms were observed under electronic gastroscope,and the adverse reactions during the treatment were recorded.Statistical software was used to analyze the results.Results:1.A total of 148 patients were included in this study,including 74 patients in the treatment group,including 2 patients who lost follow-up(1 patient lost follow-up,1 patient self-discontinued),and the final case was 72 patients.There were 74 cases in the control group,1 case was removed,1 case was shed,and the final case was 72 cases.2.Comparison of efficacy of TCM syndromes: compared with the control group,the treatment group was superior to the control group in the improvement of epigastric pain,weakness of limbs,cold and coldness of stomach,spit and clear saliva,and loss of appetite(P<0.05).There was no significant difference in the improvement of loose stool between the two groups(P>0.05).The treatment group was superior to the control group in improving the total score of TCM symptoms(P<0.05).The effective rate of the treatment group was 94.44%,and that of the control group was 84.72%.The curative effect of the treatment group was better than that of the control group(P<0.05).3.Comparison of eradication rate of Hp: 72 cases in the treatment group,69 cases of Hp conversion to Yin,the eradication rate was 95.83%.In the control group,there were 72 cases,65 cases of Hp transvulsion,and the eradication rate was 90.28%.There was no significant difference in Hp eradication between the two groups(P > 0.05).4.Comparison of gastroscopic efficacy: in the treatment group,72 cases were cured,31 cases were cured,29 cases were demonstrated,11 cases were effective,and 1 case was ineffective,with an effective rate of 98.61%.In the control group,there were 72 cases,23 cases were cured,22 cases were effective,14 cases were effective,13 cases were ineffective,the effective rate was 81.94.The therapeutic effect of gastroscope in the treatment group was better than that in the control group(P<0.05).5.Comparison of adverse reactions: during the treatment,1 patient in the treatment group developed abdominal distension,and the incidence of adverse reactions was 1.39%.There were 7 cases of adverse reactions in the control group,including 1 case of nausea,1case of abdominal pain,2 cases of abdominal distension,2 cases of diarrhea,and 1 case of constipation.The incidence of adverse reactions was 9.72%.The treatment group had fewer adverse reactions than the control group(P<0.05).6.Comparison of safety: after treatment,there were no obvious abnormalities in blood,urine,stool routine,liver and kidney function and electrocardiogram of the two groups of patients.Conclusion:Xiangsa lizhong decoction combined with rabeprazole in the treatment of hp-related ulcer(spleen-stomach asthenia cold type)has an accurate clinical efficacy.The combined treatment of traditional Chinese and western medicine can promote the healing of ulcer and improve the symptoms of traditional Chinese medicine,which has certain clinical promotion value. |
