The Treatment Of The Tabial Plateau Fractures

Objective: To increase the cure rate and decrease the disabilities rate of tibial plateau fractures by studying the treatment of the tibial plateau fractures and analyzing the clinical effectiveness.Methods: The 136 cases were retrospectively studied. There are 85 males (62.5%) and 51 females (37.5%) with an average age of 39 years. All the fractures are fresh and one side. According to Schatzker Type System, there were 21 typeâ… ( 15.4%) , 32 typeâ…¡(23.5%), 22 typeâ…¢(16.2%), 15 typeâ…£(11.1%0,19 typeâ…¤(13.9%),27 typeâ…¥(19.9%). In all the associated injuries, there are 25 accessory ligament (18.4%), 26 cruciate ligament (19.1%), 35 meniscus (25.7%), 18 vascular nerve (13.2%), 29 multiple fractures (21.3%). The 10 cases were expectant treatments and the rest were operative treatments. Most of the operative treatments were open reduction and internal fixation. The patients systemically started the rehabilitation and took the measures against the complications.Results: The 136 cases were followed from 8 to 38 months. According to Rasmussen Knee Function Grading System, the results were excellent: 90 cases (66.1%), good: 19 cases (14.0%), fair: 18cases (13.2%), poor:9 cases (6.6%). the cure rate is 80.2%. The curative effect of the tibial plateau fractures was concerned with the fracture type. By the X2 test (X2=17.67>X20.05,5=11.07),P<0.05,The curative effect of typeâ… was the best and typeâ…¥was the worst. In these cases, there were 31 cases(22.8%) that occurred the complication. There were 17 cases traumatic arthritis(12.5%),6 cases anchylosis(4.4%),5 cases sinus tract(3.7%).Conclusion: Prognosis is dependent to the type of fracture. The doctors must take the preoperatively clinical and imageology assessment, especially in the muscle and skin injury. The doctors can select the best operative opportunity in order to avoid cutaneous necrosis, internal fixation exposure and sinus tract. In the operation, the doctors should be aware of soft tissue protection and how to fix. The doctors should pay more attention to high-energy fracture. In the post-operation, the functional exercise is indispensable.In this way, the cure rate of tibial plateau fractures will be increased and the disabilities rate will be decreased.
