Objective: To investigate the expressions of Survivin and C-myc and relationship with the clinical features of hilar cholangiocarcinoma and the correlation between the expressions of Survivin and C-myc.Methods: The expressions of Survivin and C-myc in 45 cases of hilar cholangiocarcinoma and in 20 cases of non-carcinoma bile duct tissues were examined by immunohistochemistry assay. Laboratory data were then analysed statistically together with the related clinical-pathological data.Results:(1) The expression of Survivin in hilar cholangiocarcinoma: In 45 cases of hilar cholangiocarcinoma, Survivin was expressed in 31 cases, the positive rate of Survivin expression was 68.9% (31/45) , the positive rate of Survivin in 20 cases of non-cancerous bile duct tissues was 0 (0/20) , the difference was significant(P<0.05 ) .The expression of Survivin in hilar cholangiocarcinoma was related with lymph node metastasis. The expression of Survivin in cases with lymph node metastasis was higher than that in cases with no lymph node metastasis (P<0.05) . However the expression of Survivin had no association with the Bismuth type(P>0.05) , age and gender (P>0.05) , tumor size (P>0.05) .(2) The expression of C-myc in hilar cholangiocarcinoma: In 45 cases of hilar cholangiocarcinoma, C-myc was expressed in 28 cases, the positive rate of C-myc expression was 62.2% (28/45) .In 20 cases of non-cancerous bile duct tissues, C-myc was expressed in 3 cases, the positive rate of C-myc expression was 15% (3/20) ,the difference was significant (P<0.05) .The expression of C-myc was related with differentiated degree and lymph node metastasis(P<0.05 ). However the expression of C-myc had no association with gender, age, tumor size and the Bismuth type(P>0.05) .(3) The correlation between the expressions of Survivin and C-myc in hilar cholangiocarcinoma: Of the 31 Survivin-positive cases, C-myc was expressed in 24 cases. Of the 14 Survivin-negative cases, C-myc was expressed only in 4 cases. the difference was significant (x~2=9.79 P<0.01) .Conclusions: ( 1 ) The expressions of Survivin and C-myc in hilar cholangiocarcinoma were significantly higher than that in non-cancerous bile duct tissues. The expressions of Survivin and C-myc had no association with the Bismuth type ,gender, age, and tumor size. However the expressions of Survivin and C-myc were related with lymph node metastasis. (2) The expression of Survivin was significantly correlated with the expression of C-myc in hilar cholangiocarcinoma(P<0.01) . (3) Joint action of Survivin and C-myc may play partial roles in oncogenesis,development, metastasis and prognosis of hilar cholangiocarcinoma. Survivin is respected to be a new therapeutic target/pathway of hilar cholangiocarcinoma.
