Objective: To investigate the expression of hMLH1.hMSH2 and its relationship with the clinical features of hilar cholangiocarcinoma.Methods: In 54 specimens of hilar cholangiocarcinoma, 25 specimens of normal bile duct tissues, the expressions of hMLH1.hMSH2 were detected by immunohistochemistry methods. Laboratory data were then analysed statistically together with the related clinical-pathological data.Results: (1) In 54 cases of hilar cholangiocarcinoma, hMLH1 was expressed in 24 cases, the positive rate was 44.4%. In 25 cases of normal bile duct tissues, hMLH1 were detected in 23 cases, the positive rate was 92%. The expression of hMLH1 in hilar cholangiocarcinoma was obviously reduced, the difference had significance(P<0.05). The expression of hMLH1 in hilar cholangiocarcinoma had no association with age and gender (P>0.05) , the Bismuth type (P>0.05) , but the expression was related with lymph node metastasis(P<0.05)and pathological stage (P<0.05) . (2) In 54 cases of hilar cholangiocarcinoma, hMSH2 was expressed in 21 cases, the positive rate was 38.9%. In 25 cases of normal bile duct tissues, hMSH2 were detected in 21 cases, the positive rate was 84%. The expression of hMSH2 in hilar cholangiocarcinoma was obviously reduced, the difference had significance(P<0.05). The expression of hMSH2 in hilar cholangiocarcinoma had no association with age and gender (P>0.05) , the Bismuth type (P>0.05 ) , but the expression was related with lymph node metastasis(P<0.05)and pathological stage (P<0.05) (3) The expression of hMLH1, hMSH2 had correlation with prognosis. The 2-year-survival rate of hMLH1- negative patients was significantly lower than hMLH1-positive patients after radical operation (15% VS 45.4% .P<0.05). The 2-year-survival rate of hMSH2- negative patients was significantly lower than hMSH2-positive patients after radical operation (23.5% VS 44% .P<0.05).Conclusions:(1)The expression, of hMLH1, hMSH2 in hilar cholangiocarcinoma were significantly lower than that in normal bile duct tissues. (2) The lower expression of hMLH1, hMSH2 in hilar cholangiocarcinoma indicates that it is correlated with the occurrence of hilar cholangiocarcinoma. (3) Joint action of hMLH1.hMSH2 plays an important role in the oncogenesis and metastasis of hilar cholangiocarcinoma. (4) hMLH1.hMSH2 may be valuable factors to indicate prognosis.
