Diabetes mellitus (DM) is a chronic disease and frequently encountered disease, It effects heart, brain, kidney and so on. Conventional intermediate long-acting insulins have an onset of action of 0.5-4 h, a maximal effect at 4-20h and a duration of 24h. The absorption pattern of conventional long-and intermediate-acting insulins put many patients at risk of nocturnal hypoglycaemia. Insulin glargine is a new long-acting insulin analogue. it has amino acid changes in both the A and B chain of the molecule, compared to human insulin.Objective and method: It is to observe the lowering sugar effect and safety of insulin Glargine on type 2 diabetes patients. 80 cases with type 2 diabetes between 2004. 12 and 2006. 12, diagnosed by 1999 WHO methods, were selected. 80 patents with type 2 diabetes were randomly divided into insulin Glargine group (using insulin Glargine, n=40) and NPH group (using NPH, n=40). Comparation was made between them in terms of fasting blood glucose (FBG), postprandial blood sugar, HbA1c, duration of glycemin control and hypoglycemic episodes.Observing body weight, blood fat, urea nitrogen and creatinine by using insulin Glargine from 0 week to 24 weeks.Results: By using insulin Glargline, FBG, postprandial blood sugar, HbA1c, and blood fat were significant descention from 0 week to 24 weeks (P<0.05). The hypoglycemic episode were lower in insulin glargine group than those in NPH group. The hypoglycemic episode of insulin glarine group was 1case (2.5%), NPH group was 8 cases (20%). The nighttime hypoglycemic episode of insulin glarine group was 0 case, NPH group was 3 cases (7.5%). There was a signigicant difference between the two groups (P<0.05). the body weight, urea nitrogen and creatinine by using insulin Glargine from 0 week to 24 weeks were not difference. (P>0.05).Conclusions: Insulin glargine had greater efficacy in lowering blood glucose, HbA1c and blood fat, it is also much safer than NPH insulin at hypoglycemic episode, Especially nighttime hypoglycemic episode. The effect of Insulin glargine can retain 24 hours and the peak of action was equability. By using insulin glargine, the blood glucose of patients were satisfactory.
