Evaluation Of Intrarenal Arteria Flow In Cardiorenal Syndrome Patient By Color Doppler Ultrasonography

Objective: To evaluate the value of renal haemodynamics changes in patients with cardiarenal syndrome by color Doppler ultrasonographyMethods: All the patients of chronic heard failue were divided into two groups according to blood creatinine, that is groupA (renal function is insufficiency) and group B (renal function is normal) .The maximal velocity(Vmax) and resistance index(RI)of segmental and interlobar renal arteries were examined by color dopple ultrasonography , and compared with normal group.Results: In the patients of group A, the Vmax of segmental and interlobar renal arteries were significantly lower than group B and normal group (26.40±6.06cm/sVmax of SRA vs 43.38±5.04cm/sVmax of SRA and 50.18±2.41cm/sVmax of SRA; 17.13±4.18cm/sVmax of IRA 24.43±3.61cm/sVmax of IRA and 27.96±1.79cm/sVmax of IRA) , P=0.000, the difference is statistical significance between three groups. But the RI were significantly higher than group B and normal group (0.72±0.07 vs 0.66±0.04 and 0.60+0.02 in SRA; 0.71±0.06 vs 0.65+0.04 and 0.59±0.02 in IRA), P=0.006,P=0.000,P=0.007 and P=0.000, meanwhile , the difference is statistical significance between three groups. In the patients of chronic heart failure ,RI of segmental renal arteries is positive correlation with age, course of disease, TC (total cholesterol) and SCr (serum creatinine) , correlation coefficient r=0.664, r=0.615, r=0.786 and r=0.601, P=0.007,P=0.015,P = 0.001 and P=0.018, and there is no significance correlation with SBP (systolic blood pressure) and DBP (diastolic blood pressure) , correlation coefficient r = - 0.26 and r =0.373, P=0.349 and P=0.171.A11 of patients with chronic heart failure were divided into four group ,these are group I,group II and groupIII (because the conlution of groupIV were serirs,the numerus is absence.).In the groupIII, the Vmax of segmental and interlobar renal arteries were significantly lower than group II and group I (27.01±2.28cm/sVmax of SRA vs 32.48±5.75cm/sVmax of SRA and 44.63±4.39cm/sVmax of SRA; 15.3±2.44cm/sVmax of IRA vs 20.66±3.27cm/sVmax of IRA and 26.13±2.71cm/sVmax of IRA), P=0.000.Meanwhile group II is lower than group I , P=0.032 and P=0.003. But the RI were significantly higher than group II and group I (0.749±0.04 vs 0.685±0.04 and 0.626±0.03 in SRA ; 0.774±0.003 vs 0.677±0.004 and 0.625±0.004 in IRA), P=0.004, P=0.001, P= 0.000, P=0.012.There were significantly difference of Vmax and RI of segmental and interlobar renal arteries among the three groups.Conclusion: The intrarenal arteries of cardiarenal syndrome patients were in a stste of low systolic and diastolic flow velocity ,but high renal vascular resistance. RI of segmental renal arteries is positive correlation with age,course of disease,TC and SCr.
