There were more and more patients with diabetes mellitus with the changingof habitation. Patients with diabetes mellitus were easy to have complicationof depression because of the affection of pressure and mental factor.The Sick rate was 3 percents as to the statistics of world healthorganization. But this data in patients with diabetes mellitus was 33 percents.A questionnaire study including 410 cases showed that therewas 16.6 percentsof patients with diabetes mellitus had extreme mental pressure. So thetreatment for patients with diabetes mellitus should include some treatmentfor depression. As the anti-depression western drugs, its strong side effectsled to the bad compliance. But the traditional Chinese medicine, which wasused under the guide of theories of concept of wholism and differentiationof syndromes and signs, could well adjust the body status and has low toxicity,low side effects and good curative effect. It could apparently improve thequality of life in patients with diabetic depression and had good complianceand good prospect in clinic.Our study analyzed the syndrome distribution and six channels syndromefrequency of diabetic depression with the method of clinical survey anddifferentiation of syndromes and signs. This might provide new idea for thetreatment of this disease in clinic. ObjectiveTo survey themental health status of diabetic patients and to statisticthe incidence of depression in diabetic patient. To compare the differencebetween the depression group and non-depression group in diabetic patients.To discuss the affection of depression on psychology and physiology of diabeticpatients. Then we investigated the differentiation rule of syndromes and signsin patients with both diabetes mellitus and depression. We compared thecurative effect fore and aft therapy and investigated the curative effect oftraditional Chinese medicine compound recipe on patients with both diabetesmellitus and depression.MethodsThis study used forward looking, random, single blind and control researchmethods. It divided into two parts including questionnaire survey andclinical observation. We selected about 300 cases of patientswith diabetesmellitusin specialty clinic and ward of the No.1 affiliated hospital ofGuangzhou university of Chinese medicine from July 2006 to December 2006.We collected the data and diagnosed it with method of differentiation ofsyndromes and signs. It was also differentiated by six channel syndromes.Database was built and SPSS10.0 software was used to do statistics.One-factor analysis of variance, logistic regression analysis and chi squaretest was used in this software.ResultsWe collected 300 cases with 152 male and 148 female patients with diabetesmellitus. Their age was range from 21 years old to 85 years old and the averageage was 55.8±12.3 years old. Results showed that there was no correlationbetween the depression extent and gender (P>0.05) and was correlation betweenthe depression grade and gender (P<0.01). the older the patients, the lessthe depression (P<0.01). There was no correlation between the depression andthe course of disease (P>0.05). There was apparent correlation between thedepression extent and complications (P<0.05). And the higher the fasting blood glucose, the stronger the depression extent (P<0.05). There was correlationbetween the HbAlc and the depression extent (P<0.05). The higher the bloodpressure, the stronger the depression extent (P<0.05). There was apparentdifference among different syndromes (P<0.05).Patients with syndrome of damp-heat in the spleen and the stomach,syndrome of qi-stagnation of the liver, syndrome of deficiency of both qi andyin, syndrome of extreme heat due to deficiency of yin, syndrome of deficiencyof both yin and yang and syndrome of blood stasis was 62, 108, 26, 40, 10 and24 cases respectively.Syndrome of qi-stagnation of the liver was the main syndrome for patientswith diabetic depression. As to the location of the disease, the liver, heart,lung, spleen, kidney, large intestine, small intestine, stomach, gallbladder,sanjiao and bladder took 292, 245, 226, 269, 277, 158, 126, 141, 135, 155 and141 cases respectively. This showed viscera was the most location of thedisease.As to six channels differentiation result, single channel disease tookthe most that was 265 cases. For complicated diseases of shaoyang and yangming,complicated diseases of taiyang and shaoyang, complicated diseases of taiyangand yangming and complicated diseases of taiyang and shaoyin, there were 4cases, 27 cases, 2 cases and 2 cases respectively. As to statistics, the caseswith complicated disease, complication and double-channel febrile diseasewere also included into the single channel disease. Results showed that thetaiyang disease, yangming disease, shaoyang disease, taiying disease, shaoyindisease and jueyin disease took 108 cases, 71 cases, 99 cases, 135 cases, 119cases and 158 cases respectively. This showed that yin channels took morecases.We treated 30 cases in clinic. There were 13 male and 17 female patientswith diabetic depression. The data was analyzed with the method of Riditanalysis. There was apparent improvement in symptoms and signs after treatment(P<0.05). There was apparent improvement in HbAlc, fasting blood glucose, blood pressure and blood fat (P<0.05).ConclusionIn one word, our study showed that the main pathogenesis for this diseasewas liver gloomy. The excess syndrome was most qi-stagnation of liver,damp-heat in the spleen and blood stasis. The deficiency syndrome was mostdeficiency of both qi and yin, extreme heat due to deficiency of yin anddeficiency of both yin and yang.The course of disease was changing from the excess syndrome to thedeficiency syndrome. Thus the treatment should centre on this pathogenesis.
