Part 1 The research of voiding function of BladderObjective: 1. Compare residual urine with residual fraction in the evaluation of bladder outlet obstruction.2. Compare residual urine with residual fraction in the evaluation of the function of detrucer muscle.3. The relationship between PVR and the total volume in bladder before voiding, The relationship between RF and the total volume in bladder before voiding.4. Compare the reliability of residual urine and residual fraction in repeat test.Material and Methods:1. Patients: A total 56 patients aged from 52 to 84 year-old who suffered from BPH were included in this study. The average age (61.66±6.97)year-old.2. Urodynamic investigation: The uroflowmetry was carried out firstly to get maximum flow rate (Qmax). The catheterization was performed to measure post-voiding residual immediately after voiding, residual fraction was calculated at the same time. Then fill the bladder, Detrusor instability (DI)were observed. Patients were told to void when they could not delay it. The pressure of bladder and flow rate were record. A-G number was calculated as detrusor pressure at Qmax (Pdet at Qmax)-2Qmax. WF factor were calculated by computer. The methods, definitions and units conform to the standards proposed by the International Continence Society (ICS).3. Statistical analysis: All numerical value were expressed in the form of (?)±s, Spearman correlation analysis were used to analyze the correlation between PVR and AG number, RF and AG number, PVR and WF, RF and WF. 24 patient's PVR were measure two times, relation index and variable range were used for evaluate repeat reliability.Result:1. The coefficient correlation between PVR and AG value was0.552, the coefficient correlation between RF and AG value was0.704, the coefficient correlation between WF and PVR was-0.580, the coefficient correlation between WF and RF was-0.640, the coefficient correlation between PVR and total volume in bladder before voiding was0.537, the coefficient correlation between RF and total volume in bladder before voiding was 0.088。2. The coefficient correlation in repeat test of RF was better than PVR and the variable range of PVR in repeat test was larger than RF.Conclusion:1. Both of post-voiding residual and residual fraction are related with obstruction, the latter is obvious.2. Both of post-voiding residual and residual fraction are related with the detrucer muscle function, the latter is obvious.3. Post-voiding residual is obviously influenced by the total volume in bladder before voiding while there are no relationship between the post-voiding residual and the total volume in bladder before voiding.4. The reliability of residual fraction is better than post-voiding residua in repeat test.5. Residual fraction could be consider to be one indicaton of operation in BPH patient.Part 2 the factor contribute to IPSSObjective: To explore the factors contribute to IPSS.Material and Methods:1. Patients: A total 56 patients aged from 52 to 84 year-old who suffered from BPH were included in this study. The average age was (61.66±6.97).2. the urodynamic test was done as same as part 1.3. International prostate symptom score (IPSS) were used in our study, the questionnaire were filled by patients under doctor's instruction.4. Statistical analysis: indepent sample test and logistic regression were used, all calculations were done by SPSS 12.0 statistical software.Result: 1. bladder outlet Obstrction, the function of bladder and unstable bladder are three factors contribute to International prostate symptom score.Conclusion:1. The symptom of Benign prostatic obstrction patients are determined by bladder outlet Obstrction, the function of bladder and unstable bladder.2. we should prudent when the BPH patient ask for operation without obvious obstrction but only for relieve symptom.
