Objective:To observe the influence of aprotinin on celite-basedactivated clotting time(C-ACT),kaolin-based activated clottingtime(K-ACT) and whole blood prothrombin time(BPT) incardiopulmonary bypass during cardiopulmonary bypass(CPB).Materials and Methods: 21 patients were selected who hadaccepted cardiopulmonary bypss heart surgery in our division duringJanuary to February,2007,of which 12 patients(experiment group) hadbeen give aprotinin during CPB and 9 patients(contrast group) hadn't. Theaprotinin which dose was 50,000KIU/kg was all added to the primingsolution. Collected blood specimens to masure C-ACT, K-ACT and BPTprior to heparinization,5 minutes after heparinization and 30 minutes afterCPB.Then compared C-ACT, K-ACT and BPT between experiment groupand contrast group, and analysed the influence of aprotinin to C-ACT,K-ACT and BPT.Result:1.C-ACT The C-ACT of experiment group and contrast groupprior to heparinization was 114.5±11.4 seconds and 108.8±11.2 senonds,p=0.265, no statistics significance; 5 minutes after heparinization was568.4±50.9 seconds and 559.3±70.7 seconds, p=0.735, no statistics significance; 30 minutes after CPB was 1055.7±271.4 seconds and703.7±179.7 seconds, p=0.003,had statistics significance, there wasobvious difference between experiment group and contrast group,itmanifested that the aprotinin prolongs C-ACT during cardiopulmonarybypass.2.K-ACT The K-ACT of experiment group and contrast groupprior to heparinization was 115.7±7.0 seconds and 110.7±11.5 seconds,p=0.233, no statistics significance;5 minutes after heparinization was682.1±170.6 seconds and 656.6±112.2 seconds, p=0.702,no statisticssignificance; 30 minutes after CPB was 1261.2±270.2 seconds and920.0±287.4 seconds, p=0.012,had statistics significance, there wasobvious difference between experiment group and contrast group,itmanifested that the aprotinin also prolongs K-ACT duringcardiopulmonary bypass.3. BPT The BPT of experiment group and contrast group prior toheparinization was 14.8±1.0 seconds and 14.7±0.9 seconds, p=0.800,no statistics significance;5 minutes after heparinization was 40.5±7.3seconds and 39.3±4.0 seconds,p=0.662,no statistics significance;30minutes after CPB was 43.2±6.7 seconds and 41.5±4.2 seconds,p=0.527,still had no statistics significance, there was no obviousdifference between experiment group and contrast group after aprotininused,it manifested that the aprotinin didn't effect BPT during during cardiopulmonary bypass.Conclusions:1. Aprotinin prolonged activated clotting time(ACT) obviouslyduring cardiopulmonary bypass whether the activator is celite or kaolin.2. Aprotinin didn't effect the whole blood prothrombin time(BPT)during cardiopulmonary bypass.
