Background and ObjectiveTo study the relationship between ultrasonic signs and patho-manifestation and expressions of PCNA in HCC, and approach the value of HCC biological behaviour estimated by ultrasonic features.Materials and methodsThe samples of this study were from 41 patients with HCC proved by operation or pathology. Before operationin , they were examined with 2D and color Doppler ultrasonography in order to observe the lump's size, bouncary, amicula, echogram and state of blood flow in the hepar. Meanwhile, it was also obseved to detect whether there was carcino-epistom in the portal vein or and the area next to great blood vessels,and to estimate by fractionation the blood supply of the tumor. ,and estimate lump's echogramMethods of immunity tissue chemical staining including S-P and H-E were used to determine the PCNA expressions through nyxis biopsy or specimen excision after operation to analyze the relationship between ultrasonic signs and patho-manifestation and expressions of PCNA in HCC.Results(1) PCNA expression rates in poorly differentiated HCC were obviously higher than those in middle or highly differentiated HCC; and PCNA expression rate increased more obviously with carcino-epistom in the portal vein and lymphatic gland metastasis than those without carcino-epistom in the portal vein and lymphatic gland metastasis. (P<0.05)(2) .Ultrasound proved that PCNA expression rates were obviously higher in HCC with the tumor's largest diameter more than 3 cm than those in HCC with the diameter less than or equal to 3cm,and PCNA expression rate increased with the augmentation of the tumor's volume. (P<0.05);(3) .PCNA expression the central necrosis of the tumor rates were obviously higher in HCC with blurred tumor, non halo sign, ,profuse blood supply ,portal vein tumor thrombus than those with clear tumor and without the central necrosis of the tumor , profuse blood supply ,portal vein tumor thrombus . (P<0.05) There was no evident difference of PCNA expression rates in different inner echograms. (P>0.05)(4) Ultrasonic signs showed that, with the tumor more than or equal to 3cm, HCC with blurred contours,the central necrosis of the tumor, profuse flows, carcino-epistom in the portal vein and lymphatic gland metastasis led to lower-grade histological differentiations than what HCC with clear contours and without the central necrosis of the tumor,profuse flows, carcino-epistom in the portal vein and lymphatic gland metastasis led to. (P<0.05) There was no evident difference of histological differentiations in different inner echograms. (P> 0.05)Conclusions(1) The PCNA expression was negatively correlated with the HCC differentiation, and positively correlated with carcino-epistom in the portal vein and lymphatic metastasis of HCC.(2) Degree of PCNA expression can be acted as one effective index to reflect HCC's proliferating state,malignancy and potential possibility of tumor metastasis.(3) In the HCC's ultrasonic signs,there was certain relativity between PCNA expressions and such things, as the size of the tumor,the bouncary echogram ,the blood supply,whether there was central necrosis ,whether there was carcino-epistom in the portal vein and whether there was lymphatic gland metastasis.(4) Ultrasonic signs had a meaningful role in estimating HCC's malig biological behaviour.
