A Clinical Study Of Three Kinds Of Active IUD Inserted Immediately After Induced Abortion

OBJECTIVE: To assess the clinical effects, side effects, safety and acceptability of TCu380AIUD,γ-IUD and UCDCu200IUD insertion immediately after induced abortion and select a scientific foundation fit for the parous women in our country.METHODS: A prospective, random clinical study was carried out. One hundred and fifty healthy, parous and the first trimester pregnant women aged 20-40 were recruited from gynecologic clinics of the first affiliated hospital of Kunming Medical College from March 2005 to September 2005. They were randomly divided into three groups Every group included 50 cases which received one of three kinds of IUD insertion immediately after induced abortion by special cultural physician. Follow-up was arranged at 1, 3, 6, and 12 months after insertion. The discontinuation rates were calculated by life table method. SPSS 11.5 was used for analyzing the side effects and Logistic regression model for analyzing probable influence factors of contraception efficacy of three kinds of IUD.RESULT: At the end of the first year, there was no pregnancy occurred in the three groups. The expulsion rate was 8.0, 2.0, 14.0/100 cases, respectively. The expulsion rate was lower inγ-IUD group than in other two groups, but no statistical significant (P＞0.05). The cause of removal rate was 2.0, 0.0, 10.0/100 cases, respectively. It was higher in UCDCu200 group than in TCu380A andγ-IUD group, but no statistical significant (P＞0.05). The continuation rate was 88.0, 90.0, 72.0/100 cases, respectively. It was significant higher in TCu380A andγ-IUD group than in UCDCu200 group(P＜0.05). In the side effects, the incidence of menorrhagia was 2.3, 0.0, 2.6/100 cases respectively, no statistical difference was shown(P＞0.05). The rate of irregular bleeding occurrence is 0.0, 0.0, 2.4/100 cases respectively. It was higher in UCDCu200 than in other two groups, but no statistical difference was shown (P＞0.05). The incidence of prolonged menstruation was 7.0, 24.4, 34.2/100 cases respectively. It was significantly lower in TCu380A group than in other two groups(P＜0.05). The incidence of pain was 13.6, 4.4, 21.1/100 cases respectively, but no statistical difference was shown in three groups(P＞0.05). One case of pelvic inflammatory disease(PID) was observed in UCDCu200 group.CONDLUSION: As excellent contraception efficacy, little complication, lower expulsion rates, higher continuation rates, little side effects on TCu380AIUD,γ-IUD and UCDCu200, which were inserted immediately after induced abortion, they were recommend to wide spreading use.
