Study On Hydrosalpinx Of The Effect On The Outcome Of IVF-ET And Endometrial Receptivity

Objective: The effect of hydrosalpinx on the outcome of IVF-ETwas studied with retrospective analysis. And to investigate the expressionof endometrial CFTR, OPN and integrinβ₃ in women with/withouthydrosalpinx, we explored the relationship between this infertility andendometrial receptivity.Methods: The first part of this research: A total of (Groupâ… ) 143IVF-ET cycles with hydrosalpinx were evaluated, and compared with (Groupâ…¡) 263 IVF-ET cycles with tubal diseases but no hydrosalpinxfrom August 2003 to August 2005. The difference in the outcomes ofIVF-ET between two groups were analysed; The second part:Endometrial samples were sucked from 71 women (31 patients withhydrosalpinx, 29 patients with tubal obstruction but no hydrosalpinx and11 women from control group) in the implantation window beforeIVF-ET from May to August 2005. Paraffin-embedded tissue sectionswere analyzed by S-ABC immunohistochemistry technique for the CFTR,OPN and integrinβ₃ expression, which was quantitated by imageanalysis in the context of histopathological parameters. All data wereanalysed by software engineering SPSS 12.0.Results: 1. The clinic pregnancy rates (32.87%) and implantationrates (18.61%) of Groupâ… were much lower than Groupâ…¡, P＜0.05.And the early pregnancy loss rates (19.15%) and ectopic pregnancyrates (17.02%)of Groupâ… were significantly higher than Groupâ…¡,P＜0.05.2. The pathological examination results showed that the proportionof phase no significant difference among groups, P＞0.05.3. The strong staining of CFTR was found in the membrane andcytoplasm of lumianal and glandular epithelial with hydrosalpinx.Thepatients of hydroysalpinx exhibited a significantly lower averagegray-scale values of CFTR ( 103.98±24.11) compared to either group,P＜0.05.4. OPN was significant found in plasma of gland and cavityepithelium cell in all endometria. And the average gray-scale values were no marked difference among three groups, P＞0.05.5. Integrinβ₃ was significantly found in plasma of gland and cavityepithelium cell.In the three groups, the average gray-scale valuewere 161.42±63.05,121.58±43.08,109.70±46.50. And the absence ofendometrial integrin is lot of the patients of hydraysapinx.Conclusions: 1. Tubal disease accompanied by hydrosalpinx reducedsuccessful rates of IVF-ET.2. CFTR, OPN andα_vβ₃ integrin were detceted in endometrialepithelium during the window of implantation.These might be importantmodulators of the endometrial receptivity and the process of embryoimplantation.3. The upregulation of CFTR expression and the absence andreduction ofα_vβ₃ integrin expression in endometria with hydrosalpinxduring implantation phase may be important causes to reduce thereceptivity of the endometrium and the successful rates of IVF-ET.4. The expression of OPN in endometria during the window ofimplantation might have no significant relevance to endometialreceptivity with hydrosalpinx.
