Purpose:To study the expression of TGF-β1 in the spleen of the patients with portal hypertension and the influence of splenectomy on TGF-β1 levels in the blood serum of the patients with portal hypertension.Method:16 cases with portal hypertension ,7 cases Child-Pugh grade A (Child-Pugh A group) and 9 cases Child-Pugh grade B(Child-Pugh B group) ,and 7 cases of spleen injury(Normal control group) were treated by splenectomy in our hospital from May 2006 to February 2007.The TGF-β1 levels in the serum blood were detected preoperatively and ten days postoperatively by ELISA method. The TGF-β1 expressions of the spleen were detected by immunohistochemistry method,and IOD for TGF-β1 were measured by CMIAS8 computer colour image analysis systems..Result:(1).TGFβ-1-positive stained cells were recognized in the spleen non-parenchima of patients. IOD for TGF-β1 in the spleen in different group were 39.05±7.07 in normal group, Child-PughA (77.15±11.05), Child-PughB (95.39±11.35)。The results indicated that with the development of hepatic fibrosis ,the expression of TGF-β1 in the spleen was significantly increased.(2).The levels of serum TGF-β1 in different group before splenectomy and 10 days after splenectomy respectively were 49.89±7.41 (ug/L),48.93±7.49(ug/L) in normal group, 91.99±12.50 (ug/L),73.03±7.72(ug/L) in Child-PughA group,and 129.22±10.84(ug/L), 110.954±11.36(ug/L) in Child-PughB group. The levels of TGF-β1 in blood serum was significantly increased with the development of hepatic fibrosis and the levels of TGF-β1 in blood serum after splenectomy in Child-Pugh A group and Child-Pugh B group was significantly reduced than the ones before operation.Conclusion:(1).With the development of hepatic fibrosis,the expressions of TGF-β1 in the spleen were significantly increased.(2). The levels of TGF-β1 in blood serum after splenectomy were significantly lower than which before splenectomy by inhibiting the production of spleen-derived TGF-β1.
