Objective To study on the effects of early intervention on premature infants development and the serum levels of insulin-like growth factor-1 and growth hormone, to explore the clinical value of early intervention to premature infants'physcical development, And to study the relationship between insulin-like growth factor -1, growth hormone and weight ,height, mental and sychomotive development .Methods 60 cases of survived premature infants, gestational age from 32 to 36weeks and birth weight 1000-2499g preterm births from JUL 2004 to JAN 2006 in the Second Affiliated Hospital of Nanhua University, excluding those with congenital deformity ,hereditary metabolic diseases and serious complication,were randomly adivided to 2groups: experimental group (30cases) and control group(30cases). Two groups received the same treatment , routine care and feeding in hospital. In the experimental group were discharged form hospital, The parents were instructed to cultivate the infants cognition, language, emotion and communication ability, and the infants were given massage, subjected to exercise and received active motor training according to unified intervention content. All infants with abnormal motor manifestations were given appropriate rehabilitation training. In control group, infants just received similar routine care. At the age of 3months,6months, and 12months, all infants in the two groups received infant development tests with Child Development Center of China (CDCC)scale,including mental development index and psychomotive development index, and height and weight were also measured . The examiner did not know which infant had received intervention. Using ELISA technology, the serum level of insulin-like growth factor-1(IGF-1) and growth hormone(GH) were detected at the age of 3days, 3months and 6months.At the age of one year old,all infants were evaluated by nervous system examination. all infants'brains paralysis formation rates were calculated.Results 1.The results of two groups at the age of 3months,6months and 12months showed that the average scores of weight and height were significantly higher in experimental group than those in control group, the differences were significant(P<0.01). 2.At the age of 3months, the average scores of MDI and PDI in the experimental group were higher than those in control group , the differences were significant(P<0.05);At the age of 6,12months, the average scores of MDI and PDI in the experimental group were higher than those in control group , the differences were extremely significant(P<0.01). 3. At the age of 12months,the incidence of cerebral palsy in the experimental group was 10%,that in the control group was 33.3%. There were significantly difference between the two groups (P<0.05). 4.The serum levels of IGF-1 and GH on 3th day after birth were not different between the two groups (P>0.05). At the age of 3months, the serum levels of IGF-1 in the experimental group were higher than those in control group(P<0.01). ,but the serum levels of GH were no different between the two groups(P>0.05). The serum levels of IGF-1 and GH on 6 months in the experimental group were higher than those in control group(P<0.01) . 5.The relationship between insulin-like growth factor-1 , growth hormone and weight ,height, mental and sychomotive development :The levels of serum IGF-1 on 3 months after birth were positively correlated with weight, height and were not related to MDI and PDI. The levels of serum GH were not related to weight, height, MDI and PDI(P>0.05). The levels of serum IGF-1 and GH on 6 months were correlated positively with weight, height ,MDI and PDI(P<0.05). 6.Between relevance of IGF-1 and GH : The levels of IGF-1 were not related to GH on 3 months (r=0.11,P=0.41),but at the age of 6 months the levels of serum IGF-1 were correlated positively with GH(r=0.50,P=0.00). Conclusion 1. Early intervention may promote premature infants development and reduce the incidence of cerebral palsy and improve human's quality. 2.Early intervention may increase the levels of serum IGF-1 and GH and promote the central nervous system and enhance their growth 3. IGF-1 play a main role in the regulation of late growth and development. GH-IGF-1 axis is formed on 6 month after birth.
