Intralesional Glucocorticoid Therapy For Infantile Hemangiomas And GC Influence On The Expression Of The VEGF,bFGF,PCNA In The Hemangioma's Tissues

Objective:To study the clinical effectiveness of intralesional glucocorticoid therapy for infantile hemangiomas in proliferative stage,and its influence on the expressin of VEGF,bFGF,PCNA in the hemangioma's tissues.Method:26 patients(age<3months)with strawberry hemangioma received intralesional Triamcinolone Acetonide injections (The dose of injection was 1mg/cm2 per time,but not exceeding 2mg/kg). Three to five injections were given,spaced two to four weeks apart.Another way,25 patients(age range 85 days to 8 months)with hemangioma in proliferative stages and 5 patiens (age>1year)with hemangioma in involuting stage were divided into two groups. Experimental group: 5 infantile hemangiomas in proliferative stage were not been injected GC;5 cases were been injected Triamcinolone Acetonide 3 days ago of operation,5cases were been done 6 days ago of operation, 5 cases were been done 9 days ago and 5 cases were been done 2 weeks ago.Control group: 5 infantile hemangiomas in involuting stage did not received therapy.The speciman of the hemangioma were collected to detect the expression of VEGF,bFGF,PCNA by immunohistochemistry.Results:26 hemangiomas's hyperemia were lessen after one or two injections,and some of the hemangiomas were softening and flatting after 4 or 5 injections.Observing the specimen dyed by HE in the light microscope ,there were no significant difference between injected group and non-injected group.But by the means of immunohistochemistry, the expression of the VEGF,bFGF could been down regulation by the glucocorticoid,the relationship of the effection time of the GC and the exptression of the VEGF,bFGF was obvionsly negativ correlation(3d>6d>9d>2w,P<0.05),and the coefficient correlation of VEGF,bFGF were -0.5922(P<0.01),0.7402(P<0.01). The expression of the PCNA was not influenced by GC in two weeks(P>0.05).Conclusion: Intralesional corticosteroid therapy for infantile hemangiomas is safe ,reliable and convient,which ias able to greatly decrease the experrssion of VEGF,bFGF,PCNA over 2 weeks,and have no significant influence on the expressionof PCNA in 2 weeks.
