Objective 1.to study the correlation between sudden deafness and immune function 2.to study the correlation between sudden deafness and the white blood cell(WBC) levelMethods This experiment consists of two parts.The first part is to determine the serum immune globulin and the subpopulation groups of T lymph cell of ninety-three patients with sudden deafness using immune turbidimetry and flow cytometry (FCM).The results are contrasted to the reference value of twenty-three health volunteers .Then giving them decaesadril two weeks later, the serum immune globulin and the subpopulation groups of T lymph cell of the patients suffered from sudden deafness are examined.The second part is to divide the above patients with sudden deafness into two groups according to the counts of WBC.The patients with sudden deafness whose counts of WBC more than ten thousand per nanoliter and /or the percentage of NEUT(neutrophils)more than seventy-five belong to the high WBC group and the other whose counts of WBC less than ten thousand per nanoliter and percentage of NEUT less than seventy-five belong to the normal WBC group.The high WBC group is given peripheral blood hemoculture.The two groups are provided energy,neurotrophy nutriceutical,pimobendane,hormone, vitamin et al,one time per day and the high WBC group is given antibiotics simultaneously.After two weeks the counts of WBC of the patients with sudden deafness are rechecked.Result:The experimental results consisted of two sections: Section one:The T lymphocyte subset(CD3+,CD4+,CD8+,CD4/CD8) of patients with sudden deafness were 51.89±11.32%,27.09±9.17%,24.67±7.19% and 1.42±0.47 respectively.CD3+,CD4+ levels of patients were lowered,compared to the control group and the laboratory reference range(P<0.05).The serum immune globulins(lgA,IgG,IgM) and the complements (C3,C4)were 2.45±0.89(g/L),13.16±3.11(g/L),1.23±0.51(g/L),1.20±0.16(g/L),0.49±0.18(g/L)respectively. Compared with the control group,IgG level of patients was higher(P<0.05),while lgA,IgM and C3,C4 levels of patients were normal(P>0.05).After treatment with steroid therapy, The Immunoglobulin and T lymphocyte subset levels of patients went up to the normal(P<0.05). Section two:WBC level in 41 patients was higher than that in remaining 52 patients. The hemocultures of high WBC level group were negative,not found any microbes. There was no significant difference of deafness between two groups(P>0.05),but the therapeutic effect for the patients with higher WBC level was better than those with normal WBC level(P<0.05).There was a significant difference in the levels of WBC and NEUT between the two groups before treatmentt(P<0.01), but not after treatment.Conclusion 1.These results demonstrate significant abnormalities in the subpopulations of T lymPhocytes(CD3+,CD4+) and serum immune globulin(IgG) in patients with sudden dealness.It suggests that the existence of immune-mediated response in the inner ear should be possible etiopathogenic factor in this entity.2.The WBC counts in nearly 1/2 patients with sudden deafness were higher. The therapeutic effect was achieved more rapidly in the group with a higher WBC level, indicating that a higher WBC was a good prognostic sign for sudden deafness.While the cause of WBC rising was unknown,it needs us to study more.
