Study On The Dependablity Of Hyopthyroidism And Renal Function

Objects:To investigate the relations between thyroid function and renal function in patients with primary hypothyroidism.Methods:serum creatine(Cr),uric acid in serum(Ua),creatinine clearance(CC),serum urea nitrogen(Bun),urinary albumin determine nature,free triiodothyronine(FT₃), free thyroxin(FT₄),sensitive thyrotropin(sTSH)were measured in 45 patients with primary hypothyroidism and 30 healthy subjects.Results:Cr,Ua,and urinary albumin determine nature positive in patients were significantly higher than in healthy subjects(P＜0.05)and CC in patients was lower than healthy subjects(P＜0.05).FT₃,FT₄ and sTSH had no relation with Bun.But FT₃ was positive related with CC(r=0.5441,P＜0.05)and negatively related with Ua(r =-0.4372,P＜0.05);Cr and urinary albumin determine nature positive in female patients were significantly higher than in male patients(P＜0.05)howerer CC in female patients was lower than male patients(P＜0.05);CC in the patients of over 51 years old was lower than the other different age groups(P＜0.05);CC in the patients who course of disease was more than two years was lower and urinary albumin determine nature positive was higher than the the patients whose duration was less than two years groups(P＜0.05);CC in the patients of typical hypothyroidism was lower and and Cr,urinary albumin determine nature positive were higher than the patients of subclinical hypothyr-oidism and earlier peroid groups(P＜0.05).Ua in the patients of subclinical hypothyroidism was lower than the other groups(P＜0.05);Cr,Ua,urinary albumin determine nature positive in the patients of post-treatment were lower and CC was higher than the patients of pretherapy(P＜0.05);there were 3 cases in 6 patients with primary hypothyroidism and maybe with primary renal disease,whose the pathologic type of renal are membranous nephropathy.Conclusion:Cr,Ua,urinary albumin determine nature positive are increased and CC is lower in patients with primary hypothyroidism,and the lower level of FT₃ was responsible for the lower levels of Cr and higher levels of Ua;Renal function is damaged more obviously in female hypothyroidism patients than male patients;The degree of renal function damage is different in different hypothyroidism age groups.it is the most obviously in over 51 years old group;Renal damage is relevant with the course of hypothyroidism.Renal damage is the most obviously in the patients whose course of disease exceed two years and the damage become serious with the course;The degree of hypothyroidism is more serious,the renal damage is more obiously;The renal damage induced by hypothyroidism is reversiability.It can be returned with the hypothyroidism treatment.The membranous nephropathy maybe be the most common pathologic type of hypothyroidism renal damage.
