Objective: To investigate the relationships between expression of VEGF,infiltration of DC and clinicopathologic parameters in hepatocellular carcinoma and their effects on postoperative recurrence and metastasis.Methods: The expression of VEGF and density of tumour infiltrating DC(TIDC) in 103 cases of hepatocellular carcinoma tissues were detected by immunohischemistry technique (S-P method).S-100 protein was used for the marker of TIDC. The relationships between the expression of VEGF,the density of TIDC and the clinopathological parameters (including HBsAg ,the serum level of AFP,the status of capsule,the degree of differentiation,tumour thrombus,the diameters of tumour focus, the status of curative resection and metastasis without liver ) and their effects on postoperative recurrence and metastasis were analyzed.Results: (1)Spearman correlation analysis showed that the expression of VEGF was correlated with the density of TIDC (p=0.000,r=-0.437) and the diameter of tumour focus (p=0.020,r=0.229) .(2) There were two negative correlations between the density of TIDC and the expression of VEGF (p=0.000,r=-0.437) , and the diameter of tumour focus (p=0.040,r=-0.203) ,while both the expression of VEGF and the density of TIDC were not correlated with the serum level of AFP,the status of capsule,the degree of differentiation,tumour thrombus, the status of curative resection and metastasis without liver. (3)HBsAg was negatively correlated with the expression of VEGF. (4)Life Table survival analysis showed that mean disease-free survival time in the low expression of VEGF group(23.465±2.876 month)was not remarkably longer than that in the high expression of VEGF group(18.222±3.232month ,p=0.166). But Life Table showed that the survival curve in the low expression of VEGF group was much higher than that in the high expression group, which indicated that the disease-free survival time in the former one was longer than that in the latter one. The 1 year disease-free survival ratio in the low expression of VEGF group(81.4%)was significantly higher that in the high expression group(55.6%, P=0.013). (5)Mean disease-free survival time in density of TIDC positive group(26.220 months) was significantly longer than that in density of TIDC negative group (15.526 months , P=0.002) . (6)Multivariate Cox proportional harzard model analysis showed that the density of TIDC ( PO.003, B=-1.019) was independent factor which could effect the postoperative recurrence and metastasis.Conclusion: (1) TIDC, representing the local immune status of the host, was an independence contributive factor. It may play an important role in immunological defense mechanism of host versus tumour. Increasing number of TIDC can enhance the disease-free survival time of HCC resected . The expression of VEGF perhaps increased the risk of postoperative recurrence and metastasis. (2)In hepatocellular carcinoma tissues the expression of VEGF and density of TIDC was correlated negatively. VEGF may inhibit the proliferation and development of DC. The expression of VEGF was also positively correlated with the diameter of tumour focus and there was relationship between the density of TIDC and the diameter of tumour focus,while both the expression of VEGF and the density of TIDC was not correlated with the serum level of AFP,the status of capsule,the degree of differentiation,tumour thrombus, the status of curative resection and metastasis without liver. (3) HBV adjusted negativelythe expression of VEGF by some mechanisms.
