Objective : To study the effect of focused ultrasound on the morphous and cleaning function of nasal mucosa.Methods :â‘ A model CZB ultrasound therapeutic system for rhinitis was used in this study. The nasal mucosa of inferior turbinate of sheep was exposed to focused ultrasound, and morphological changes of the nasal mucosa were observed by gross examination, light microscope, transmission electron microscope and scanning electron microscope.â‘¡we selected the inferior turbinate nasal mucosa and established a culture of nasal mucosa respiratory epithelial cells in vitro with the method of mechanical splitting and trypsinization. By this way, we could observe the change of biology characteristics of nasal mucosa epithelial cells between prior-scanning and post scanning.â‘¢We selected 45 cases from 168 cases of allergic rhinitis patients who were received ultrasound therapeutic and measured the nasal mucociliary clearance time with saccharin test between 30min prior- scanning and 30min post- scanning, observed the statistical significance of NMCT with the coupled T test.Results:â‘ There was no obvious change in appearance and color of the treated nasal mucosa in the inferior turbinate of the sheep right after scanning. At the 3rd day after scanning, the epithelial lamina of nasal mucosa was intact and there was diffused distribution of spot coagulated necroses in the subepithelia layer of treated nasal mucosa under light microscopic. Degeneration of vascular endothelial cells and thrombosis of blood vessels were found in the coagulated necrosis region. Besides, degeneration or necrosis of nerve cells was induced, and glandular cells were partially or completely damaged. The structure of epithelial goblet cells, ciliated cells and cellula columnoepithelialis were normal under the observation of scanning electron microscope. Besides, the ultramicrostructure of cilia and tight junction appeared to be no damage and the cell nucleus and mitochondria of endothelial cells were intact under the observation of transmission electron microscope. At the 7th day after scanning, signs of tissue regeneration such as hyperplasia of collagen fiber were found in the coagulated region, and the necrosis tissue began to dissolve and was absorbed at the 14th day post-scanning.â‘¡We could establish a culture of nasal epithelial cells in vitro with the modified trypsinization method. This method could obtain a great quantity of nasal epithelial cells, elevate the quantity and the activity of cells, avoid the injure of epithelial cells with Dispaseâ…¡and shorten the time of cell primary culture. We also discovered that there was no obvious change in the growth of nasal mucosa epithelial cells, the beating of cilia and the secretion function of beaker cells in therapeutic dose focused ultrasound.â‘¢There was no significant difference in NMCT between 30min prior-scanning and 30 min post- scanning with coupled T test(P=0.893>0.05).Conclusion:â‘ when the therapeutic dose focused ultrasound was function at the nasal mucosa, The energy could be deposited specifically at the subepithelia layer, ablate the targeted nasal mucous tissues which were rich in blood vessels, nerves and glands. There was no damage on the non-target area such as the epithelial layer and no effect on the biology characteristics and the cleaning function of nasal mucosa epithelial cells. So the focused ultrasound was a micro-insult treatment to nasal mucosa subepithelia layer.â‘¡The modified enzyme digestion of a culture of nasal mucosa epithelial cells in vitro which was quick, efficient and simple.
