Objective: To investigate the dynamic changes of IL-1βand sE-selectin in serum of patients with acute pancreatitis(AP)and their significance,and study their value in diagnosis of severe acute pancreatitis(SAP)and predic-tion of outcome of AP.Methods: Forty one patients with acute pancreatitis were divided into severe acute pancreatitis (SAP,19 cases) group and mild acute pancreati-tis(MAP,22 cases)group.Their serum were collected on the 1st, 3rd, 7th, and 14th (SAP) day after admission respectively and the serum IL-1βand sE-selectin concentrations were determinated with ELISA . 20 healthy volun-teers were taken as controls.Results: 1.The serum IL-1βconcentration was markedly elevated in AP.The peak is on the 1st day(SAP:86.91±13.32 pg/ml,MAP:63.18±7.47 pg/ml).The serum levels of IL-1βwere declined on the 3rd, 7th and 14th (SAP) day after admission . The serum IL-1βlevels of SAP were markedly higher than those of MAP and normal controls on the 1st,3rd, 7th day (p<0.05 )and it decreased close to normal on the 14th day(p>0.05). The serum IL-1βlevels of MAP were markedly higher than those of normal controls on the 1st,3rd day (p<0.05 ) and it decreased close to normal on the 7th day(p>0.05).2. The se-rum sE-selectin concentration was markedly elevated in SAP. The peak is on the 3rd day(95.65±13.06 ng/ml).The serum levels of sE-selectin were de-clined on the 7th and 14th day. The serum sE-selectin levels of SAP were markedly higher than those of MAP and normal controls on the 1st, 3rd, 7th day (p<0.05) and it decreased close to normal on the 14th day (p>0.05). The sE-selectin level was also elevated in MAP and the peak was on the 1st day(41.60±6.85 ng/ml).The serum levels of sE-selectin were declined on the 3rd and 7th day .The serum sE-selectin levels of MAP were markedly higher than those of normal controls on the 1st,3rd day (p<0.05 ) and it decreased close to normal on the 7th day(p>0.05).3. There was a positive correlation between the serum level of IL-1βand sE-selectin in AP on the 1st day after admission(r=0.851,p<0.01).Conclusion: The dynamic changes of IL-1βand sE-selectin in serum are closely correlated with the clinical progression of AP. The levels of IL-1βand sE-selectin in serum may be served as effective markers in early SAP diagnosis and observing its course of progression . The combinated determine of IL-1βand sE-selectin may be used as one index to judge the prognosis.
