Object : To discuss the effect of nitroglycerin in the rat severe acute pancreatitis model,s plasm ET/NO , TXA2/PGI2 and pancreas morphological changes,to find the nitroglycerin,s mechnism in the severe acute pancreatitis,to provid theoretic evidence for clinic application. Methods: In order to study the mechanisms of microcirculatory disorder in rats with severe acute pancreatitis and the intervener of nitroglycerin,Sixty S-D rats were selected and divided randomly into five groups:including normal control group(A group,n=12),severe acute pancreatitis group(B group,n=12) and nitroglycerin groups: 0.5ug/kg/min, 1ug/kg/min,2ug/kg/min,30min ,(C ,D, E group,n=12). SAP models were established by injecting 4% sodium taurocholate(STc) into the main pancreatic duct under the surgery. The rats of C,D and E group were intravenously injected nitroglycerin 0.5ug/kg/min , 1ug/kg/min ,2ug/kg/min,continously for 30 minutes at 5 minutins and 6th hour and 12th hour respectively after severe acute pancreatitis induction.The changes of indexes that thromboxane A2(TXA2), prostaglandin I2(PGI2), endothelin(ET) and nitric oxide (NO) were dynamically determined after operations,the AMY and pancreas morphological changes in the synchronization time point were evaluated and graded.Results:A,B,C,D and E groups AMY at 6th hour(3131.00±708.76)U/l,(15048.67±661.66)U/l,(11711.67±538.76)U/l,(12087.33±746.06)U/l,(10326.50±711.58)U/l,12th hour(2585.43±524.72)U/l,(15507.00±756.78)U/l,(11370.67±664.51)U/l,(9794.00±426.43)U/l,(8887.83±489.94)U/l;at the synchronization time points,nitroglycerin intervene group compare the AMY to B group were decline and P value<0.05;A,B, C,D,E group plasma TXB2/6-keto-PGF1a ratio at 6th hour point were(1.04±0.02),(2.17±0.21),(1.93±0.07),(1.78±0.13),(1.67±0.18),12th hour were (1.04±0.02),(2.52±0.12),(2.12±0.21),(2.00±0.07),(1.91±0.07);at the synchronization time points,nitroglycerin intervene group compare the plasma TBXB2/6-keto-PGF1a ratio to B group were decline and P value<0.05;the ET/NO ratio at 6th hour were(0.86±0.05),(1.03±0.07),( 0.88±0.03 ),( 0.85±0.05 ),( 0.82±0.02 ), at the 6th time point,nitroglycerin intervene group compare the ET/NO ratio to B group were decline and P value<0.05;at 12th hour were (0.83±0.03),(1.23±0.04),(1.16±0.07),(1.17±0.08),(1.11±0.04),A,E group compare the plasma ET/NO ratio to B group were decline and P value<0.05,but B,C group compare the ET/NO ratio to B group were decline and P value﹥0.05. A,B,C,D and E groups pancreas morphological scores at 6th hour point were(0.58±0.37),(8.21±1.44),(6.00±0.44),(6.08±0.73),(5.33±0.68),12th hour were(0.58±0.37),(12.75±1.69),(10.00±1.18),(9.00±0.77),( 8.41±0.66 ); at the synchronization time points,nitroglycerin intervene group C,D and E pancreas morphological scores compare to B group were decline and P value<0.05。Conclusions:⑴nitroglycerin can depress SD rat SAP pancreas morphological damage,and the effect has dosage relationship,in some degree.⑵nitroglycerin can nitroglycerin can improve the ration of plasm ET/NO ,and make it tend to normol balance; nitroglycerin can improve the ration of plasm ET/NO ,and make it ternd to normol balance;and the effect has dosage relationship,in some degree.⑶nitroglycerin can improve the ration of plasm TXA2/PGI2,and make it ternd to normol balance; and the effect has dosage relationship,in some degree.
