Objective:To study the anti-conjunctival bleb scarring formation about Panax notogins- eng saponins in rabbits after filtration surgery.Methods:Trabeculectomy was performed on 72 eyes of 36 healthy adult rabbits which were randomly divided into 3 groups(every group have 24 eyes of 12 rabbits), experimental group:performed trabeculectomy using PNS.control group1:performed trabeculectomy using NS.control group2:performed trabeculectomy using MMC. intraocular pressure(IOP),filtering bleb,reaction of the anterior chamber and complica- tions were performed On 1,3,7,14,30 days after surgery respectively.Two rabbits selected randomly in every group were killed on days 1,3,7 and 14 after surgery.Tissue was harvested from the bleb area and was made pathological section,then was H-E stained and immunostained and observed under light microscope.TGF-β1 Protein expression and the number of fibroblast were quantitative analyzed by image analysis technique,the toxicity of PNS was observed with electron microscope.The data was analyzed statistically by SPSS 12.0 for windows.Results:⑴IOP: low IOP sustained in PNS group longer than in NS group.⑵The filtering blebs: diffuse filtering blebs were observed in PNS group.⑶Complication: there was not any complication in PNS group.⑷Pathological section was observed under light microscope: the number of fibroblast counted by computer showed significant difference between PNS group and NS group(P<0.05),and no obvious difference with MMC group(P>0.05).⑸TGF-β1 expression in the fibroblast endochylema: buffy grains in the fibroblast endochylema under light microscope was considered as positive signal,the intensity of the color reaction in PNS group was weakly positive,while the intensity of the color reaction in NS group was strongly positive.there was significant difference in the expression of TGF-β1 between PNS group and NS group(P<0.05)at day 3 after surgery.⑹Observed under electron microscope: at day 7 after surgery,the corpuscular ultrastructure in PNS group and NS group was normal.while the corpuscular ultrastr- ucture in MMC group was destroied.Conclusion:PNS obviously inhibited the expression of TGF-β1 of fibroblast and conjunctival scarring,maintained the lowering of IOP with clinical safety and nontoxicity. PNS can be regarded as an adjuvant agent to reduce subconjunctival scarring after filtration surgery.
