Objectives: To evaluation the expression of cathepsin B in the transitional cell carcinorma of bladder(BTCC) and its correlation with the recurrence of carcinoma.Methods: 172 tissues were collected from 40 BTCC patiences who had been recurred for 3 times or more (3 times is contained) in the urology department of hua xi hospital of si chuan university from 1994 to 2006. Among the 172 pathology tissues,40 were primary carcinorma,40 were first recurrence, 40 were second recurrence,52 had been recured for 3 times or more;92 were low grade carcinorma,80were high grade carcinoma;98 were stage I carcinoma,74 were between stage II to IV;16 normal bladder tissue is recruited as control.The tissues were fixed by formalin and embeded in paraffin.The tissues were cut into 4 um thick fragment,one is for HE staining ,one is for PBS negative control,the other two is use for immunohistochemistry.The last step is micrography,image collection and statistical analysis.Results:immunohistochemistry stain demonstrates that the positive cells of cathepsin B was diffused in the cancer tissues with obvious stroma staining.The label index of BTCC tissues is obviously higher than nomal tissues'(p<0.01),and the label index is increased with the recurrence time(p<0.01).The lable index of high differentiation carcinoma is higher than that of low differentiation carcinoma; cathepsin B staining of the normal bladder tissue is negative or only a few positive cells,but in the high grade and stage carcimoma tissues ,the staining is positive and disperse.Conclutions:the cathepsin B is detected in BTCC when immune-ohistochemistry method is used.The expression of cathepsin B is significantly correlated with the stage and grade of the carcinoma and play an important role in the invasion and development of the carcinoma. Cathepsin B should be a good tool to determin the bladder tumour behaviour and its prognosis.
