The incidence of malignant tumors which has been doing serious harm to human life and health is increasing. Lung cancer is one of the most common malignant tumors currently. Half a century, the high incidence of lung cancer still has been increasing in the world.The incidence and the mortality rate of lung cancer is the first for male cancer ,second to third for female. Global annual number of deaths for lung cancer is more than breast cancer and prostate cancer combined. The situation of our country is not optimistic, the incidence and the mortality rate of lung cancer still have increasing.The number of lung squamous carcinoma, lung adenocarcinoma and small cell carcinoma are increasing obviously. Malignant tumor invasion and metastasis are the main factors for high mortality rates. Selectin family is one of cell adhesion molecules as a receptor molecule group which plays an important role in the inflammatory response, the formation of angiogenesis and the metastasis of many kinds of malignant tumors. CD24, SLeX are ligands of selectin family. Until now, many experiments at home and abroad have already showed that CD24, SLeX are high expression in the solid tumor of some organs, and their specific combination with selectin family participate in cancer invasion and metastasis. Using immunohistochemistry, the expression of CD24 and SLeX antigens were studied in 70 lung cancer organizations.We discuss the association between them with their clinicopathological parameters.Expecting to find a kind of index which can help us to judge benign or malignant, evaluate the state of illness, assess curative effect and prognosis for clinic. Providing the basis for the molecular target treatment of lung cancer.Mothod:We selected a total of 70 patients with lung cancer who visit in thoracic surgical department of China-Japan Union Hospital Affiliated Jilin University during a period from 1995 to 2007 as the research objects studied. There are 34 adenocarcinomas, 29 squamous cell carcinomas and seven small cell carcinomas. The gender distribution was 34 men and 36 women. The mean age was 58.16±8.92 years old,with the range from 40 to 72 years old.According to TNM classification of lung cancer United States and the International Union Against Cancer Committee in 2002 enacted, 47 patients in stage I to II, 23 patients in stage III. There are 32 patients whose cigarette smoking is higher than 400(year×stick). All patients with no history of chemotherapy and radiotherapy.And some patients with follow-up investigation,the follow-up time is the date of the surgery, the survival time is the time for patients leding to the death with metastasis, spreading or recurrence. We have gained the follow-up information of 54 patients.Among the total,there are 49 cases of non-small cell lung cancer.Results: 1.Expression of CD24 in Lung cancer:(1)The expression and localization of CD24 was tawny in the cellular cytoplasm and membrane,there were no expression in nucleus.We have seen varying degrees of expression in membrane and/or cytoplasm in the lung adenocarcinoma, squamous cell carcinoma.There was no expression in SCLC(seven cases of SCLC no positive response). However, duing to small-cell lung cancer specimens were little, we need further research confirm the credibility of the results.(2)The positive rate of CD24 in NSCLC was 41.3% (26/63),with staining intensity mostly≥+ + (five were +, 11 were + + ,10 were + + +). The positive rate in adenocarcinoma 55.9% (19/34) significantly higher than that in squamous cell carcinoma 24.1% (7/29), the difference was significant (P <0.05).(3)There was no correlation between the expression of CD24 with the patient's age, sex, smoking index, the degree of tumor differentiation and clinical stages in adenocarcinoma and squamous cell carcinoma, there was no significant difference (P> 0.05). 2. Expression of SLeXin lung cancer:(1)The expression and localization of SLeX was tawny in the cellular cytoplasm and membrane, there were no expression in the nucleus,ess expression in SCLC (only one case of SLeX is positive in seven cases of SCLC, the positive area <10%).However, duing to SCLC specimens were little, we need further research confirm the credibility of the results.(2) The positive rate of SLeXin NSCLC was 63.5% (40/63) , with staining intensity mostly≥+ + (nine were +, 18 were + + ,23 were + + +)。The positive rate in lung adenocarcinoma was 76.5% (26/34), significantly higher than that in squamous cell carcinomas 48.3%(14/29), there were significant differences (P <0.05).(3)The patients with pulmonary adenocarcinoma, squamous cell carcinoma in stage III, the positive rate of SLeX was 83.3% (15/18), higher than that in stage I,II 55.6% (25/45), poorly differentiated lung cancer 80.8% (21/26) was higher than that in well differentiated lung cancer 51.4% (19/37), the positive rate of SLeX in female patients 76.7% (23/30) than that in male patients 51.5% (17/33), there were significant differences (P <0.05). 3. An association between CD24 and SLeX was found of NSCLC. r=0.2763,P=0.0165(<0.05)。4. The patients with positive expression of CD24 whose average survival time was significantly lower than that with negative expression of CD24. The patients with positive expression of SLeX whose average survival time was significantly lower than that with negative expression of SLeX,the difference was significant (P <0.05).The patients with positive expression of both CD24 and SLeX whose mean survival time was significantly reducer than that with the one positive antigen and two negative antigens. There was significant difference (P <0.05).On the whole, we can conclude: The expression of SLeX in the stage III of pulmonary adenocarcinoma,squamous cell carcinoma was significantly higher than that in the stage I,II,and The expression of SLeX in poorly differentiated lung cancer was higher than that in the well differentiated lung cancer.The result suggests that the SLeX has relation with the tumor grade and stage in NSCLC;Comparing the concentration of CD24 and SLeX in the patients with lung squamous carcinoma and lung adenocarcinoma, the above two indicators of patients with lung adenocarcinoma are higher and the results have statistic significance.We presume that maybe it has something concerned with adenocarcinoma is abundant with blood vessel, which avail metastasis,and its malignancy is relatively higher than squamous cell carcinoma; In addition, an association between CD24 and SLeX was found of NSCLC. Whether SLeX or CD24 with positive expression in patients,and their average survival time was significantly lower than negative expression,and the average survival time with positive expression of CD24 is obviously lower than positive expression of SLeX.The average survival time of the patients with positive expression of both antigens is the lowest. It suggests that those antigens might have synergistic effect in the development of NSCLC.With the research of expression of CD24 and SLeX in lung cancer(squamous cell carcinoma and adenocarcinoma mainly) suggests that CD24 and SLeX in tumor occurrence and development play an important role and can be used as new indicators for the prognosis and targeting treatment for lung cancer.The treatment purpose that control the lung cancer metastasis to aim directly at CD24,SLeX and the corresponding selectin.It also set out for the occurrence and metastatic mechanism of lung cancer to provide some theoretical basis.
