The Expression And Clinical Significance Of Survivin And PTEN In Renal Clear Cell Carcinoma

IntroductionRenal tumor is the most common malignancy in genitourinary cancers only second to bladder tumor and 0.4%～3%of all urogenital tumor.Renal tumor mainly include renal clear cell carcinoma(RCCC,60%～85%),granular cell carcinoma and undifferentiated cancer.Studying on genes related to RCCC has great value for exploring its pathogenesis,diagnostic standard and treatment of renal cancer.Survivin is a new member of inhibitor of apoptosis proteins,which was fist separated from the library of Human Genome Project's hybridization selection by yale university's Altieri,using the receptor of effector cell protease receptor-1.Many aspects of research showed that Survivin induced the loss of the tumor cell's apoptosis,it is closely related with the tumor's occurrence,its expressions always positive related with tumor's malignant degree and poor prognosis.PTEN gene is the first found tumor suppressor gene to have lipid phosphatase activity up to now.Its whole name is phosphatase and tensin homology deleted on chromosome ten.It's not only manifestations the characteristic of suppressor gene,but also have phosphatase activity,and has been implicated in a variety of cellular functions including proliferation and apoptosis,metastasis and adhesion.PTEN gene has important effect in formation process of tumor,and has closely relationship with the growth and development of tumor.PTEN gene as a new type of suppressor gene,regulates the growth and apoptosis of cell,effects the infiltration metastasis of tumor cell and the assembly of cytoskeletal proteins has important function in signal transduction and other biological processes.Discussing the expression of PTEN and Survivin in RCCC and the relationship between them,providing the theoretical basis for the pathogenesis and diagnosis of RCCC,and discusses the significance in prognosis evaluation of RCCC preliminary.ObjectiveBy studying the expression of Survivin and PTEN in RCCC,evaluate the function of Survivin and PTEN which in the occurrence and development of RCCC and the relationship with the clinical and pathological factors of RCCC.Hoping to be helpful to reveal the function of Survivin and PTEN in the progression of RCCC,and useful to providing the theoretical basis of preventing and controlling the occurrence development and prognosis of RCCC clinically.Materials and Methods1.Materialsthis study included 89 patients with RCCC which from 2005 to 2006 in Shengjing hospital affiliated to China Medical Unversity.2.Immunohistochemistryin 89 patients,Survivin and PTEN protein's expression were detected by SP.3.CorrelationAll the data are analyzed with SPSS for Windows 13.0 software,analysis correlations between Survivin and PTEN in clinic pathologic parameters of RCCC.ResultsThe Survivin stain located mostly on cytoplasm,partly on nucleus,having no expression on membrane.The positive rates of Survivin were higher than renal tissue and cancer adjacent tissue.The PTEN stein located mostly on cytoplasm,few on membrane,none was expressioned on nucleus.In 89 pieces of RCCC tissue,the positive expression of PTEN was 40(44.9%);In 10 renal normal tissue and 18 adjacent tissue,the positive rates was 100%and 89%respectively.The expression of Survivin and PTEN was no significant correlation with the age and sexual of patient's and also with the size of tumor(P＞0.05).But the expression of PTEN inⅠ～Ⅱstage was significant higher thanⅢ～Ⅳstage(P＜0.01),and also higher in the group without lymph node metastasis than those in the group of lymph node metastasis(P＜0.01). The expression of Survivin inⅠ～Ⅱstage was significant lower thanⅢ～Ⅳstage(P＜0.05),and also lower in the group without lymph node metastasis than those in the group of lymph node metastasis(P＜0.05).There was a negative correlation between the expression of PTEN and Survivin in RCCC(r=-0.689,P=0.000).Conclusion1.The abnormal expression of Survivin may be closely related to the pathogenesis and development of RCCC,it's expression is closely relates to the malignant degree. Survivin may be the important molecular index infects the prognosis of RCCC.2.The decrease expression of PTEN may be related to the differentiation,clinical stage and metastasis of RCCC.It suggested that the tumor suppressor gene PTEN may play an important role in the pathogenesis and development of RCCC.The detection of its protein can be an objective index judging the invasive and metastatic ability of RCCC.3.There was a negative correlation between the expression of PTEN and Survivin in RCCC.The imbalance expression of them may be a reason of the original and developing mechanism of RCCC.
