| Objective To evaluate the clinical value of transrectal ultrasound guided systematic prostate biopsy on prostate cancer diagnosis. Methods 238 consecutive men with suspected prostate cancer were included in the study. In these candidates age was between 50 years and 86 years, mean 68.7 years; There were 2 cases'TPSA less than 2.5ng/ml, 14cases between 2.5～4.0ng/ml, 123 cases between 4.0～10.0ng/ml,99 cases over 10.0 ng/ml. All of the 238 cases were executed TRUS guided prostate biopsy.We reviewed the clinical data of them, contrasted TPSA,prostate volume,PSAD,(f/t)/PSAD,prostate calcification between benign prostate disease and prostate cancer, calculated the cancer detection rate for each biopsy core. Result 59 cases were diagnozed prostate cancer. Clinical stage: T1:0 cases, T2:8 cases, T3:22 cases, T4:29 cases. We find that there were significance differences between prostate cancer and benign diseases in TPSA,prostate volume,PSAD,(f/t)/PSAD,prostate calcification, and the results were as follows: benign disease: 12.5(0.97～54.4) ng/ml,70.1(12.3～206.6) cm~3,0.23(0.01～2.39)ng/ml.cm3,1.685(0.071～9.938),6.7%,16.2%;prostate cancer: 82.5(2.6～490.5) ng/ml,47.6(17.2～110.6)cm~3,2.5(0.05～22.0)ng/ml.cm~3,0.381(0.008～1.71),33.9%,49.2%. There was no difference in each biopsy core .Bleeding was the common complication ,and there were no serious complications in 238 cases. Conclusion Transrectal ultrasound guided systematic prostate biopsy, which, is easily operated, have high sensibility and scarce serious complications. If analysised TPSA,PSAD,(f/t)/PSAD,prostate calcification, the specificity would be elevated. In a word , transrectal ultrasound guided systematic prostate biopsy is worth to be generalized.
